Objective:①Theoretical discussion:To analyze and summarize the current research on the pathogenesis of liver draining and breast nodules, and to propose a new understanding of the pathogenesis of liver draining and breast nodules.②Clinical trial:To investigate the correlation between inward and outward personality traits and differences in bilious and depressive temperament types and the changes in the condition of patients with breast nodules;and to observe the clinical efficacy of the topical preparation "Breast Nodules" in the treatment of patients with breast nodules(class 3/4a,the same below),so as to provide a new method for the personalized treatment of breast nodules.Methods:① Theoretical investigation:Using the literature content analysis method,we searched authoritative databases such as China Knowledge Network,Vipshop,Pubmed, etc.,and collected primary literature with high reliability as the study sample. We used the content analysis method to summarize and organize the specific contents of the literature, analyzed the statistical data quantitatively and qualitatively, clarified the relationship between liver dysfunction and the pathogenesis of breast nodules, and proposed a new understanding of the pathogenesis of liver dysfunction and breast nodules.② Clinical trial:Patients with breast nodules who met the inclusion criteria were screened, signed an informed consent form, and completed the form face-to-face using the scale Eysenck Personality Test, and data were collected on site.Based on the results of the scale, the outward-looking and outward-looking groups of breast nodules were selected as case groups for observation and prospectively followed up for 6 months, with changes in breast nodule classification as the main outcome index.Patients with breast nodules were divided into outward and inward groups, and were treated with the topical preparation "breast nodule elimination" for 4 months, and the patients in both groups were followed up with changes in symptoms and signs and imaging examinations before,2 months and 4 months of treatment, respectively.SPSS 26.0 software was used for statistical analysis.Results:①Theoretical discussion:a total of 46 papers were included.Among the nine main pathogenic mechanisms, the loss of drainage of the liver was closely related to the pathogenesis of breast nodules, and it was considered that the lack of emotional and moral control, the abnormal drainage of the liver, too much drainage and not enough drainage were the main pathogenic mechanisms of breast nodules. In the literature, the pathogenesis of modern medicine is mostly related to endocrine disorders, hormonal disorders, relatively high estradiol(E2)and low progesterone(P),and disorders in the physiological regulation of the hypothalamic-pituitary-ovarian axis(HPOA).②Clinical trial:a total of 57 patients with treated breast nodules were included, with 70%being extroverted and 30%being introverted according to the results of the scale measurement. The differences in the color ultrasound BI-RADS classification of patients before and after observation were analyzed, and the results showed that the differences between the two groups were statistically significant(p<0.05),and the color ultrasound classification of the extroverted group was higher than that of the introverted group. The results of logistic regression analysis of the correlation between extroverted and introverted temperament types and age and changes in BI-RADS classification before and after observation showed no significant differences(p>0.05).Within-group comparison:patients in the extroverted and introverted groups showed significantly lower improvement in breast distension, lump texture, and systemic concomitant symptom scores than before treatment(p<0.05),while both groups showed no statistically significant differences in improvement in lump extent and lump size scores(p>0.05).There was no statistically significant difference in the maximum diameter of nodules between the two groups after treatment compared with that before treatment(p>0.05).The color ultrasound BI-RADS classification decreased in the inward group after treatment compared with that before enrollment(p<0.05).Comparison between groups:There was a statistically significant difference in improving the mass texture between the two groups after treatment(p<0.05).There was no significant difference in breast distension, lump size, lump extent and systemic concomitant symptom score(p>0.05).There was a statistically significant difference in BI-RADS between the two groups after treatment(p<0.05).The inward group was more effective in improving the classification of nodules. And there was no significant difference in the maximum diameter of the nodules in the post-treatment ultrasound between the two groups(p<0.05).Conclusion:①The physiological function of the liver, which is responsible for draining,is closely related to the pathogenesis of breast nodules,in which the liver, which is responsible for draining and regulating emotions and Qi and blood,is the main physiological mechanism affecting the pathogenesis of breast nodules. The loss of liver circulation is the most basic pathogenesis of breast nodules. The pathogenesis of modern medicine is endocrine and hormonal imbalance, mainly manifested by the increase of E2,decrease of P and the imbalance of E2/P ratio.(2)The main personality characteristic of patients with breast nodules is extroverted biliousness, and the ultrasound BI-RADS classification level of extroverted patients is generally higher than that of introverted patients. The new topical formulation of "Breast Nodules" was effective in treating patients with breast nodules, improving their symptoms and signs such as breast swelling and pain, texture on palpation, reducing the diameter of nodules, and reversing possible malignant changes.
