| Objective:The purpose of this study was to analyze the spinal sagittal plane parameters of patients treated with thoracolumbar kyphoplasty based on the "holistic view of Traditional Chinese medicine" by retrospective analysis and imaging data,and to explore the effects of kyphoplasty on secondary fractures of patients with osteoporotic vertebral compression fractures.Clinical research combined with traditional basic theories of Traditional Chinese medicine provides reference for the prevention and treatment of postoperative secondary fractures in patients with osteoporosis compression fracture.Methods:1.This study cases are collected in the affiliated hospital of shandong university of TCM spinal orthopedic ward,selection in November 2019-November 2020,the affiliated hospital of shandong university of TCM for line osteoporotic vertebral compression fractures of vertebral body shape(convex)after surgery patients,in strict accordance with the inclusion criteria and exclusion criteria selection conforms to the patients as the research object.The patients were followed up for 1 year after surgery.According to the follow-up results,the patients were divided into two groups:group A patients with secondary fracture after surgery;Group B included patients without secondary fracture.There were 31 patients in group A,including 4 males and 27 females.There were 31 patients in group B,including 5 males and 26 females.2.Guided by the holistic view of Traditional Chinese medicine and based on the overall spine study,the selected cases were measured with the parameters of spine-pelvis sagittal plane balance after surgery,and the results of the two groups were statistically analyzed.Full-length anteroposterior and lateral radiographs of the spine in the natural standing position were taken,and sagittal spino-pelvic balance parameters were measured on the radiographs,including:Sagittal vertical axis(SVA),Thoracic Kyphosis(TK),thoracolumbar kyphosis(TLK),lumbar lordosis(LL);Pelvic parameters included:sacral Slope(SS),pelvic tilt Angle(PT),pelvic incidence Angle(PI).Compare and analyze the differences between the two groups of data.Results:1.Comparison of general baseline data between the two groups,such as sex ratio(P=0.254),age(P=0.568),bone mineral density(P=0.811),amount of bone cement injection(P=0.613),operation duration(P=0.277)and operation segment(P=0.508),showed no statistical significance(P>0.05);2.The preoperative and postoperative 2dvas scores of group A(fracture group)were statistically significant(P<0.05),and the preoperative and postoperative 2dvas scores of group B(non fracture group)were statistically significant(P<0.05).The postoperative2 dvas scores of the two groups were not statistically significant(P>0.05);3.The comparison of PT(P=0.254)and TLK(P=0.439)between the two groups was not statistically significant(P>0.05);4.The comparison of SVA(P=0.040),TK(P=0.027),LL(P=0.034),PI(P=0.013)and SS(P=0.034)between the two groups was statistically significant(P<0.05).Conclusion:Spinal-pelvic sagittal balance has a certain influence on the secondary fractures after PKP,and the larger SVA and TK and the smaller LL,SS and PI are positively correlated with such fractures.Such parameters can be used as predictive indicators,and it is feasible to measure the balance parameters of the full length of the spine in patients with PKP after surgery,and to prevent re-fracture with reasonable postoperative intervention. |
