Observation On The Application Of TCM Diagnosis And Treatment Protocol For Children With Purpura Nephritis(2018 Edition) And Study Of Immune Status

Objective: Summarize and analyze the application of the 2018 edition of the Chinese Association of Traditional Chinese Medicine "Traditional Chinese Medicine Diagnosis and Treatment Plan for Children with Purpura Nephritis"(hereinafter referred to as the "Plan",the same below)and preliminarily discuss the immune status of children.Methods: A total of 118 cases diagnosed with purpura nephritis from January 2019 to December 2021 and met the inclusion criteria were collected from the Department of Pediatrics of the Affiliated Hospital of Shandong University of Traditional Chinese Medicine,and the diagnostic,dialectical,prescription and syndrome points were statistically analyzed to evaluate their compliance and efficacy of TCM symptoms.The immune status and related factors of 64 children with complete immunization index information were statistically analyzed.Result:1.A total of 118 children with HSPN were included in the study,and the compliance of Western medical diagnostic criteria was 100%,the conformity of the evidence type was 78.81%,and the total conformity of the prescription was 62.71%,of which 44 cases of Simiaosan,Beixiefenqing Decoction,and Ephedra Forsythia Red Adzuki Bean Decoction were 44 cases,which were foreign to the Scheme.The total effective rate of TCM symptom efficacy was 100%,and the difference in the integration rate of the three first prescriptions of damp heat internal invasion was not statistically significant(P>0.05).2.The median reference value of children with HSPN was statistically significant compared with that of healthy children(P<0.05).T helper cells CD3+ CD4+%,CD4+/CD8+ were statistically significantly different from the damp heat infestation group and the damp heat infestation group(P<0.05),and Ig M in the humid heat invasion group and the spleen and kidney yang deficiency group were statistically significant(P<0.05).The differences between total T lymphocyte CD3+ and T helper CD3+ CD4+ in the isolated hematuria group and hematuria and proteinuria were statistically significant(P<0.05),and CD4+/CD8+ were statistically significant in the isolated proteinuria group and hematuria and proteinuria(P<0.05).The differences between total T lymphocyte CD3+,T helper cell CD3+ CD4+,and total B cell CD19+ were significantly different between the abnormal group of D2 and the normal group of D2(P<0.05).Conclusion:1.The diagnostic compliance level of the included cases is consistent;the dialectical compliance level and the prescription conformity grade are relatively consistent.In all cases,the efficacy of TCM syndrome points was effective.The application of the Scheme is in good condition.2.The immune response of children with HSPN is closely related to the evidence of damp heat internal invasion,hematuria and proteinuria types,and D2 polymer levels.
