Objective:This study intends to explore the internal relationship between the distribution of TCM syndromes and quality of life in patients with peri-menopausal metrorrhagia,so as to clarify the key pathogenesis,indicate the evolution of the disease,understand the health status of the quality of life of the patients,and judge the influence of quality of life on the occurrence and development of patients’ condition.From the point of view of "treatment of pre-disease" in Chinese medicine,it intends to achieve the goal of prevention before disease and prevention of disease.It also provides theoretical support for guiding clinical diagnosis and treatment and improving the quality of life of patients.Methods:Based on the literature research,an epidemiological questionnaire was developed to investigate 120 peri-menopausal patients with metrorrhagia in the affiliated Hospital of Shandong University of Chinese medicine.According to the inclusion,exclusion and elimination criteria,the information of the questionnaire was collected and the database was established.The patients were divided into four syndrome types: kidney deficiency syndrome(kidney yin deficiency syndrome,kidney yang deficiency syndrome),spleen deficiency syndrome,blood heat syndrome(excess heat syndrome,deficiency heat syndrome)and blood stasis syndrome.The distribution of TCM syndrome types was analyzed,and the general condition of patients,age of menarche,history of pregnancy and birth,inducement of onset and quality of life were analyzed.Results:1.General distribution of peri-menopausal patients with metrorrhagia:(1)The average age of 120 patients with peri-menopausal metrorrhagia was 46.89 ±3.48 years old,of which 83.2% were between 41 and50 years old.(2)The proportion of professionals(such as teachers / doctors/ lawyers,etc.)was the highest in the occupational distribution,which was16.7%.(3)In the distribution of academic qualifications,junior high school and above accounted for 86.7%.(4)In the distribution of BMI,normal patients accounted for 57.5%,and overweight patients accounted for 33.33%.(5)In the distribution of past history,28.4% of the patients had a history of gynecological surgery.2.Menarche age and pregnancy distribution of peri-menopausal metrorrhagia patients:(1)In terms of menarche age distribution,13-year-old menarche patients accounted for the highest proportion,which was 27.5%.(2)In the distribution of the number of pregnancies,the patients with 0-4 pregnancies accounted for 91.6%,and the patients with 2 pregnancies accounted for the highest proportion of 35.8%.(3)In the distribution of the number of miscarriages,70.8% of the patients had one miscarriage and 23.3% had two miscarriages.(4)The distribution of contraceptive methods was mainly instrumental contraception,accounting for79.2%.3.Incidence and TCM syndrome distribution of peri-menopausal patients with metrorrhagia:(1)25% of patients had inducements such as work / life stress,mental tension,abortion,overwork,placement of IUD,cold drinking,cold,wading and so on.(2)In the distribution of primary and secondary syndromes,the most common main syndromes were bleeding volume such as collapse,dark red bleeding,thick bleeding and blood clots,accounting for69.2%,34.2%,50% and 95%,respectively.Swollen limbs,fatigue,abdominal pain refusing to press,premenstrual breast pain and lukewarm limbs were the most common secondary symptoms,accounting for 10%,11.7%,19.2%,15% and11.7%,respectively.(3)In the distribution of tongue pulse,pale tongue color,moist tongue,normal tongue body,thin white tongue coating and fine pulse were the most common tongue pulse distribution,accounting for 33.3%,81.7%,51.7%,51.7% and 34.2% respectively.(4)In the distribution of TCM syndrome types,blood stasis syndrome(34.17%)> spleen deficiency syndrome(25.83%)>kidney deficiency syndrome(25%)> blood-heat syndrome(15%).Among the four syndrome types,the distribution of patients with blood stasis syndrome,spleen deficiency syndrome and kidney deficiency syndrome was more uniform,with more patients of blood stasis syndrome and less patients of blood-heat syndrome.4.Distribution of quality of life: Peri-menopausal metrorrhagia had a great influence on the quality of life of patients.Among them,medical help dependence,appearance,attention concentration ability,friend support and medical service were the main factors affecting the quality of life of peri-menopausal patients with metrorrhagia.5.Analysis of related factors:The distribution of TCM syndrome types of peri-menopausal patients with metrorrhagia was not significantly different from the general situation,age of menarche,history of pregnancy and labor,inducement of disease,etc.,but correlated with psychological,social and environmental fields in the quality of life.Conclusion: 1.The pathogenesis of peri-menopausal metrorrhagia is mainly related to deficiency of spleen and kidney and blood stasis blocking Chongren acupoint.2.The general condition of patients,age of menarche,history of pregnancy and labor,inducement of onset and so on have no significant correlation with peri-menopausal metrorrhagia,which may be related to the small number of cases and limited collection site.3.Peri-menopausal metrorrhagia affects the quality of life of patients,suggesting the importance of alleviating clinical symptoms and pain of patients,paying attention to the quality of life of patients,and strengthening the daily management of chronic diseases.
