Clinical Effect And Mechanism Of TCM Comprehensive Therapy On Chronic Kidney Disease Stage 3-4

Research purpose:The patients with chronic kidney disease(CKD)stage 3-4deficiency of spleen and kidney and damp-heat stasis were treated by TCM comprehensive therapy,and the clinical efficacy was observed,and the influence of serum lipoprotein(A)expression level on kidney injury was discussed.In order to provide treatment ideas and programs with outstanding curative effect and characteristics of integrated traditional Chinese and western medicine for patients with chronic kidney disease.Research methods:Eighty patients who met the inclusion criteria were divided into experimental group and control group by random number table method,with 40 patients in each group.The control group was given Urine duqing granule combined with western medicine basic treatment,the experimental group was added with Perilla bean sauce decoction combined with Qingnitrogen enema and ear point pressing bean.A 12-week clinical study was conducted.After 12 weeks of treatment,clinical symptoms and laboratory indicators of the patients were collected for statistical analysis to evaluate the efficacy and safety of the two groups.Research results:(1)Renal function indexes,creatinine and urea nitrogen: after treatment,two groups of serum creatinine were significantly lower(P< 0.01),two groups of urea nitrogen were decreased(P< 0.05),and the experimental group is better than that of control group(P< 0.05);Cystatin C and glomerular filtration rate: After treatment,CYS-C was significantly decreased(P< 0.01),e GFR was significantly increased(P< 0.01),and the experimental group was better than the control group(P< 0.05).Urinary microalbumin and urinary ACR: after treatment,urinary microalbumin was significantly decreased in both groups(P< 0.01),and the experimental group was better than the control group(P<0.05).After treatment,urinary ACR was decreased in both groups(P< 0.05),but there was no statistical significance between the two groups(P> 0.05).Lipoprotein(A): After treatment,lipoprotein(A)was decreased in both groups(P< 0.01),and the experimental group was better than the control group(P< 0.05).(2)Clinical efficacy:after treatment,the total effective rate of experimental group was 87.5%,and that of control group was 75%.The effective rate of clinical efficacy of experimental group was significantly better than that of control group(P< 0.05).(3)The total effective rate of the experimental group was 87.5%,the total effective rate of the control group was 77.5%,the experimental group was better than the control group(P< 0.05);After treatment,TCM syndrome scores of the two groups decreased,and the experimental group was better than the control group(P< 0.01).(4)Safety evaluation: During the treatment,the basic vital signs of patients in both groups were normal,and no obvious adverse reactions occurred;The basic biochemical indexes of electrocardiogram,blood routine examination,coagulation function and liver function were normal.Conclusion:TCM comprehensive therapy can significantly improve the clinical symptoms and signs of patients with spleen-kidney deficiency and damp-heat stasis of chronic kidney disease stage 3-4,reduce TCM syndrome score,protect renal function,improve glomerular filtration rate,reduce urinary protein,reduce serum lipoprotein(A)concentration,and reduce complications.In conclusion,combined application of Traditional Chinese medicine and Traditional Chinese medicine on the basis of symptomatic western medicine treatment can significantly improve the degree of renal deterioration,reduce the expression level of serum lipoprotein(A),reduce the inflammatory state of the kidney,thereby reducing kidney damage,improve the quality of life of patients with CKD,with a practical and reliable curative effect.
