A Retrospective Study On The Laws Of Syndrome And Treatment Of Ruffian Fullness

OBJECTIVE: Based on the holistic view of three causes and measures to deal with them,this paper discusses the role of the pathogenesis view caused by aggravation in the pathogenesis,syndrome and treatment of suffocation.The general data and clinical data of 205 cases of spleen fullness(stomach sputum)collected were retrospectively integrated,and statistics and analysis were carried out on this basis to summarize their incidence,syndrome distribution and medication characteristics,in order to better guide spleen full prevention and treatment.Methods: A total of 205 patients who met the criteria were included in the clinic.For the general data and clinical data of the included cases,Excel spreadsheet software was used to establish the relevant database of this subject.SPSS26.0 statistical analysis software was used to process and analyze relevant data,and frequency analysis,chi-square test,Fisher’s exact test and other methods were used.P<0.05 was considered statistically significant.Results: The ratio of male to female in this study was about 2:3;the age range was 24～76 years old,with an average age of 56.09±9.978 years old,among which the middle-aged group had the highest incidence rate(75.1%);the disease course was mainly distributed between six months and three months.years(42%).The proportion of patients with polypsy disease of digestive system(57.1%).The aggravating seasons were more common in spring(37.1%)and winter(33.2%).Through the analysis of the Sitian-Zaiquan,the main Qi,and the politeness at the time of conception and birth of the patients,as well as the main Qi and politeness of the disease aggravation time,it can be seen that: the patients conceived in the sun cold water Sitian-Taiyin wet soil Zaiquan(20.5%)or People born with the sun-cold water(21.5%)are more likely to suffer from suffocation.When the main qi is the sun-cold water(25.4%),and the politeness is the Jueyin wind and wood(31.7%),the suffocation is more likely to be aggravated.On the basis of the guidelines and textbooks,according to the characteristics of spleen and fullness,combined with clinical practice,seven types of syndromes including wood-earth disharmony,spleen-stomach damp-heat,phlegm-damp internal resistance,phlegm and blood stasis,spleen-stomach weakness,mixed cold and heat,and stomach-yin deficiency are summarized.After analysis,it was found that the number of cases of wood-soil disharmony(39.0%)and spleen-stomach damp-heat(17.6%)was much higher than that of other syndromes.In order to clearly analyze the characteristics of deficiency and excess in the pathogenesis of suffocation and fullness,and classify them according to deficiency and excess,the study found that the incidence of suffocation and fullness was the highest(45.9%).The correlation analysis of gender,age,season of disease aggravation,Si Tian Zaiquan of conception time,politeness of birth time,main gas of aggravation time,politeness and false positive type found that there were significant differences between different genders and false positive type.Difference(P=0.011<0.05),contingency coefficient P=0.011<0.05,there is a correlation between gender and false syndrome;there is a significant difference between the season of disease aggravation and false syndrome(P=0.039<0.05),contingency coefficient P=0.039<0.05,so there is a correlation between the season of disease aggravation and the type of deficiency syndrome.There was no correlation between age,gestation time of Sitian-Zaiquan,politeness of birth time,main qi of aggravating time,politeness and false type.Drug statistics show that the top 20 most frequently used drugs are:Pinellia,Licorice,Atractylodes,Poria,Malt,Amomum,Coix Seed,Coptidis,Codonopsis,Guya,Baiji,Magnolia,Citrus,Bupleurum,tangerine peel,white peony root,radish seed,Cyperus officinalis,sea octopus,atractylodes.Among the traditional Chinese medicines with a frequency of use greater than or equal to 5,the top 10 types of traditional Chinese medicines are: tonic medicines,Xiaoshi medicines,damp-removing medicines,qi-regulating medicines,phlegm-relieving cough-relieving medicines,diuresis-dampening medicines,heat-clearing medicines,and external-relieving medicines,blood circulation drugs,hemostatic drugs.Among them,the properties of medicines are mainly warm,cold,and flat;the tastes of medicines are mainly bitter,sweet and acrid.,Liver is the most.CONCLUSION: The high-risk group of ruffian(stomach ruffian)is female;the age is concentrated in the middle-aged group,the course of the disease is more than half a year to three years,and most of them suffer from polyps disease of the digestive system.The season of exacerbation is mainly spring and winter.Cold,damp and wind among the six qi are the risk factors for the occurrence of fullness.On the basis of the guidelines and textbooks,according to the characteristics of suffocation,combined with clinical practice,seven types of syndromes,including wood-soil disharmony,spleen-stomach damp-heat,and phlegm-damp internal resistance,are summarized.The overall pathogenesis of ruffian fullness is a mixture of deficiency and reality,with spleen deficiency as the foundation and wood stagnation and dampness as the standard.The majority of male cases of ruffianism are true,while the majority of females are false and true;except in summer,the majority of false and true false positives are found in spring,autumn and winter.The treatment of ruffian fullness should take into account both cold and heat,attack and tonic,harmonize yin and yang,and maintain balance,so as to conform to the physiological characteristics of the spleen and stomach,the harmony of dryness and dampness,and the harmony of yin and yang.The basic disease location of suffocation is in the middle Jiao,involving the five internal organs,especially the lungs and liver.The basic idea of the treatment of fullness is to use Xiangsha Liujunzi Decoction,Pingwei San,and Chaihu Shugan San as the base,adding digestion and accumulation(such as malt,grain bud,radish),strengthening the spleen,clearing heat and removing dampness(such as coix seed).Ren),heat-clearing and dampness(such as Coptis chinensis),acid-reducing(such as sea octopus),astringing sores and promoting muscle(such as Baiji)and other drugs.
