| Objective: To investigate the clinical Observation of Dahuang Zhechong Decoction combined with Shaoyao Gancao Decoction in the treatment of non-proliferative diabetic retinopathy(NPDR)with liver and kidney yin deficiency entrapped in stasis,and to investigate its effects on patients’ best corrected visual acuity(BCVA),fundus,optical coherence tomography angiography(OCTA),and TCM evidence score,so as to provide a new method for the treatment of NPDR.Methods: Fifty-eight patients with liver and kidney yin deficiency with stasis type NPDR who met the diagnostic criteria were selected from the ophthalmology department of the Affiliated Hospital of Shandong University of Traditional Chinese Medicine from 2020 to 2022 and randomly divided into treatment group(29 eyes)and control group(29 eyes).In the control group,patients were given calcium hydroxybenzenesulfonate capsules orally,0.5g each time,3 times a day.In the treatment group,patients were given Dahuang Zhechong Decoction combined with Shaoyao Gancao decoction orally on top of the control group.Both groups were treated for 28 days as a course of treatment,and the efficacy was observed after two courses of treatment.The changes of best corrected visual acuity,fundus condition,OCTA superficial capillary plexus(SCP)and deep capillary plexus(DCP)blood flow density(VD),TCM evidence score,and TCM evidence efficacy were recorded and observed in both groups.Results: 1.There was no statistical difference in gender,age,disease duration and stage between the two groups after testing,P>0.05.2.Before and after treatment,BCVA in the treatment group i mproved significantly,from a mean of 0.33 ± 0.11 to a mean of 0.53 ± 0.17,P<0.01,the difference was statistically significant;BCVA in the control group improved from a mean of 0.35 ± 0.13 to a mean of 0.39 ± 0.14 after treatment,P>0.05,suggesting an improvement in visual acuity,but the difference was not statistically significant,and the comparison between the two groups Analysis P<0.01,the difference is statistically significant,suggesting that the improvement of BCVA in the treatment group is better than that in the control group.3.The total effective rate of fundus treatment was 68.97% in the treatment group and 31.03% in the control group,with a difference of statistical significance at P<0.05,suggesting that the treatment group improved the fundus lesions better than the control group.4.After treatment,SCPVD and DCPVD in the treatment group increased compared with before,from 34.69±3.09 and 38.76±3.69 to 38.14±3.15 and42.83±3.98,respectively,where the di fference of SCPVD was statistically significant,P<0.05,and the difference of DCPVD was significant,P<0.01;SCPVD and DCPVD in the control group changed from before The changes were not significant,from 34.38±3.16 and 37.93±3.90 to 36.10±3.16 and39.03±3.82,respectively,and the differences were not statistically significant(P>0.05);the comparison between the two groups before and after treatment was statistically different,P<0.01.5.The efficacy of TCM symptoms improved in both the treatment group and the control group before and after treat ment.The efficacy of TCM symptoms was 86.21% in the treatment group and 38.56% in the control group.The difference between the treatment and control groups was statistically significant,P < 0.01,and the improvement in the treatment group was significantly better than that in the control group.6.Safety analysis: no serious adverse events occurred in the two groups.Conclusion: The combination of Dahuang Zhechong Decoction and Shaoyao Gancao decoction has obvious clinical efficacy in the dialectical treatment of patients with non-proliferative diabetic retinopathy with liver and kidney yin deficiency and stasis,which can effectively reduce the TCM evidence score,improve the patients’ visual acuity,i mprove the fundus condition,increase the retinal blood flow density,thus improving the quality of life of DR patients and delaying the progression of disease,and has good clinical safety. |
