Objective: To Summarize HUANG Yuanyu’s pivot theory and analysis theoretical basis of treating ventricular premature beat with pivot theory,according to the theory of the ideas proposed Modified Huangqi Liujunzi Decoction(MHLD),observe the clinical efficacy of MHLD combined with Betaloc sustained Release tablets in the treatment of premature ventricular beat with deficiency of heart and spleen under the guidance of the pivotal theory,in order to provide a new idea for clinical treatment of this type of ventricular premature beat.Methods: 60 patients,mainly diagnosed as premature ventricular beats with arrhythmia,were enrolled in the Heart Disease out patient department or ward of The Affiliated Hospital of Shandong University of Traditional Chinese Medicine from September 2020 to September 2021 were randomly divided into therapy group(n = 30)and control group(n = 30).The control group was routinely treated with :Metoprolol Succinate Sustained-release Tablets 47.5mg,once a day.On the basis of the treatment method of the control group,the therapy group was additionally given MHLD,1 dose daily,water decocted,taken in the morning and evening.Results:1.Comparing the frequency of ventricular premature beats in 24 h dynamic electrocardiogram before and after treatment,it was 3744.83±1470.71 before treatment and 1252.27±864.62 after treatment in the therapy group;The control group was 3644.90±1489.26 before treatment and 2245.77±1030.97 after treatment.The total number of Premature ventricular contractions in the therapy group was better than that in the control group,and the statistic difference was significance(P<0.05).2.Comparison of Lown grading before and after diagnosis and treatment showed that the Lown grading of two groups both decreased after diagnosis and treatment,but the therapy group’s Lown grading improvement was more obvious than that in the control group,the statistic difference was significance(P<0.05).3.TCM syndrome points of the two groups were compared before and after treatment.Intra-group comparison: TCM syndrome points in the therapy group reduced after therapy,and the statistic difference was significance(P<0.05);The TCM syndrome points of the control group also decreased after treatment,with statistical significance(P<0.05).Comparison between groups:the points of each single symptom in the therapy group were lower than those in the control group after therapy,with statistical significance(P<0.05);The total symptom points of the therapy group was lower than that in the control group,and the difference was statistically significant(P<0.05).In terms of symptom improvement,the therapy group was better than the control group.The symptoms of palpitation,short of breath,chest tightness,stomach discomfort,chest pain,poor appetite and fatigue in the therapy group were significantly improved compared with the control group,and the statistic difference was significance(P<0.05).4.The comparison of the total effective rate between the two groups showed that the total effective rate of the therapy group was 90%,which was higher than that of the control group(66.67%).And the statistic difference was significance(P<0.05).Conclusion: Guided by the theory of HUANG Yuanyu’s pivot,this topic is to treat ventricular premature beat with deficiency of heart and spleen by using MHLD adjust the spleen and stomach central pivot.The results show that this approach is effective in the treatment of ventricular premature beat with deficiency of heart and spleen,and has outstanding efficacy in improving patients’ palpitation,shortness of breath,chest distress and chest pain,fatigue,stomach discomfort and poor appetite,and the therapeutic effect is better than that of western medicine alone.
