Clinical Investigation On Traditional Chinese Medicine Syndrome Elements Of Guillain-barre Syndrome

Objective:By investigating the clinical characteristics of patients with Guillain-Barré syndrome and analysing the clinical distribution pattern of Chinese medical evidence and integrated evidence,we provide theoretical support for the combined prevention and treatment of Guillain-Barré syndrome with Chinese and Western medicine.Methods:In this study,clinical data(including general information,relevant medical history,main symptoms,signs,physical and laboratory examinations)were collected from 36 patients with Guillain-Barr é syndrome,and the determination of Chinese medical evidence and the integration of evidence types were carried out,and the statistical software SPSS22.0 was used for statistical analysis of the data.RESULTS:(1)Season of onset was highest in the summer(15/36,41.67%),followed by autumn(10/36,27.78%),winter(6/36,16.67%)and spring(5/36,13.89%)in36 patients with Guillain-Barré syndrome.A total of 21 patients(58.33%)had a history of antecedent infection.(2)The frequency of spleen was highest among the pathogenic elements in 36 patients(20/36,55.56%),followed by liver(9/36,25.00%),kidney(8/36,22.22%),stomach(6/36,16.67%)and lung(3/36,8.33%).Among the pathogenic elements phlegm-dampness was the most frequent(24/36,66.67%),followed by qi deficiency(14/36,38.89%),blood deficiency(12/36,33.33%),blood stasis(12/36,33.33%),fire(heat)(10/36,27.78%),yang deficiency(8/36,22.23%),yin deficiency(5/36(13.89%).(3)The most frequent evidence in both the acute and recovery phases was in the spleen(10/26 38.64%,10/10 100%),followed by the liver(5/26,19.23%),lung(3/26,11.54%),stomach(2/26,7.69%),and kidney(1/26,3.85%)in the acute phase,and kidney(7/10,70.00%)and stomach(70.00%)in the recovery phase.70.00%),stomach(4/10,40.00%),liver(4/10,40.00%),and lung(0,0,).(4)Among the pathological elements,the most frequent elements in both the acute and recovery periods were phlegm and dampness(15/26 7.69%,9/10 90.00%),followed by fire(heat)(8/26,30.77%),qi deficiency(6/26,23.08%),blood deficiency(5/26,19.23%),blood stasis(4/26,15.38%),yang deficiency(2/26,7.69%)and yin deficiency(2/26,7.69%),followed by qi deficiency(8/10,80.00%),blood stasis(8/10,80.00%)and yin deficiency(2/26,7.69%)in the recovery period./26,7.69%),Yin deficiency(2/26,7.69%),followed by Qi frequency >30% for the common four diagnostic information,the common four diagnostic information are: numbness of the skin of the limbs,tiredness and fatigue,mucousy mouth,heavy body sleepiness,aggravation after activity,fatigue,dizziness,greasy coating,moist pulse,obese form,easy to catch cold,heavy body acidity,chronic poor food intake,bitter mouth,lazy speech,emotional agitation,impatience and irritability,prolonged lack of appetite,heavy joint acidity,thin coating,preference for warmth and dislike of cold,chest tightness,preference for hot drinks Among the pathogenic elements phlegm,the tongue is yellow and the pulse is slippery.(6)Among the 36 patients,the most common type of TCM evidence was phlegm-dampness trapping the spleen(20/36,55.56%).36,13.89%),incandescent liver fire(4/36,11.11%),depleted kidney yang(3/36,8.33%),and phlegm-heat impeding the lung(3/36,8.33%).Phlegm and dampness trapped in the spleen was the most frequent in the acute phase(12/26,45.15%),while stagnation of veins and channels(9/10,90%)and deficiency of both qi and blood(9/10,90%)were the most frequent in the recovery phase.(7)The age group with the highest frequency in the age of onset was the middle-aged group(16/36,44.45%),followed by the elderly(14/36,38.89%),the young(3/36,8.33%)and the juvenile(3/36,8.33%).The age group with the highest frequency in the acute phase was the middle-aged group(11/26,42.31%),while the rest were in the elderly group(10/26,38.46%),the juvenile group(3/26,11.54%),and the youth group(2/26,7.69%)in that order.The age group with the highest frequency of recovery was the middle-aged group(5/10,50.00%),while the rest were,in order,the elderly group(4/10,40.00%),the youth group(1/10,10.00%),and the juvenile group(0,0).(8)The anti ganglioside antibody test was perfected in 23 of the 36 patients,of whom a total of 15(65.22%)were positive and 8(34.78%)were negative for anti ganglioside antibodies.Conclusions:(1)Patients with Guillain-Barr é syndrome have the highest incidence in summer,with a history of antecedent infection in 21 cases.This may be due to the fact that summer is hot and humid,which makes it easy to develop epidemics and therefore more susceptible to external infections.(2)Patients with Guillain-Barré syndrome have the highest frequency of spleen among the pathogenic elements and the highest frequency of phlegm-damp among the pathogenic elements.This may be due to the fact that patients with the disease are often trapped by dampness in the spleen,and that the disease is aggravated by internal dampness due to the loss of transport and transformation of spleen deficiency over time.(4)The most common type of TCM evidence in patients with Guillain-Barré syndrome is that of phlegm-dampness trapped in the spleen.In the acute stage,phlegm-dampness trapped in the spleen is most common,while in the recovery stage,stasis blocking the meridians and channels and deficiency of both qi and blood are most common.This may be due to the fact that most patients start with dampness,which is trapped in the spleen and hinders the spleen’s transport and transformation,and that insufficient transport and transformation leads to deficiency of qi and blood,and deficiency of qi and blood over time leads to weak propulsion and stasis of the vasculature.(6)In the age grouping,the middle-aged group is the most common,followed by the elderly,youth and adolescent groups.This may be due to the fact that middle-aged patients are busier at work and are prone to depletion of Qi and blood,and that the functions of the internal organs have begun to decline in middle age,thus resulting in a higher number of cases.(7)The rate of positive anti-ganglioside antibodies in patients with Guillain-Barré syndrome was 65.22%.
