Objective:To explore the difference in clinical efficacy of functional exercise with two types of rehabilitation volume under the guidance of the theory of "dynamic and static balance" and functional exercise under the principle of painlessness in rehabilitation after Kirschner wire tension band for patella fractures.To analyze the clinical guiding significance of "dynamic and static balance" theory for functional rehabilitation after patellar fracture with Kirschner wire tension band.Methods:Ninety-six patients who met the inclusion criteria and underwent Kirschner wire tension band surgery in the Department of Orthopedics,Shandong Provincial Hospital of Traditional Chinese Medicine from October 2019 to October 2021 were selected.The subjects were randomly divided into three groups: treatment group A(Relatively small amount of rehabilitation group),treatment group B(Relatively large amount of rehabilitation group)and control group(painless group).The treatment group underwent early functional rehabilitation under the guidance of the theory of "dynamic and static balance",and the difference between the two groups was only in the number of rehabilitation.The control group followed the routine early active rehabilitation exercise under the AO pain-free principle.There were 32 subjects in each group.Comparative analysis of three groups of knee function score(HSS score),knee range of motion(ROM)before surgery,1 month,2 months,3 months after surgery and visual analogue scale of pain(VAS pain score)before surgery,postoperative 2 weeks,4 weeks,8 weeks of efficacy.The general condition of the affected limb at the last follow-up was compared among the three groups of patients.To analyze the differences in clinical efficacy of the three rehabilitation programs,and obtain the clinical guiding significance of the theory of "dynamic and static balance" and different rehabilitation amounts in rehabilitation after Kirschner wire tension band for patella fractures.Results:1.There was no significant difference in preoperative general information and perioperative variables among the three groups(P>0.05).2.The patients in both treatment groups were higher than those in the control group,and the difference was statistically significant(P<0.05)in HSS score and knee ROM at 1,2,and 3 months after operation.3.The patients in the two treatment groups were lower than those in the control group,and the difference was statistically significant(P<0.05).The VAS pain score at 2,4 and 8 weeks after operation.4.At last follow-up,the two treatment groups was significantly lower than the control group(P<0.05)in terms of pain in the affected limb,recovery to pre-injury exercise ability,and limited squatting activities.5.There was no significant difference between the two treatment groups in HSS score,knee ROM,VAS pain score and general condition of the affected limb at last follow-up(P>0.05).Conclusion:The theory of "dynamic and static balance" guides the postoperative rehabilitation of patella fracture with Kirschner wire tension band,which not only ensures the healing of the fracture,but also achieves good function.It is a safe and effective rehabilitation method.Within a certain range of rehabilitation amount,fracture healing may not be affected,and different rehabilitation amounts can achieve similar rehabilitation effects. |
