| ObjectiveTo understand the status of activities of daily living of the elderly in Henan Province,analyze its influencing factors,and explore the inflection point of the decline of activities of daily living,so as to provide an important basis for improving the health level of the elderly and providing more professional care services for the staff of the combination of medical and nursing institutions.MethodsIn this study,the multi-stage stratified cluster random sampling method was used to conduct a questionnaire survey on the resident elderly in Henan Province using the Barthel Index Rating Scale,the Mental State Scale and the Social Participation Scale.The descriptive analysis,chi-square test and binary Logistic regression were used to analyze the status and influencing factors of activities of daily living among the elderly in Henan Province.The ROC curve of age prediction of activities of daily living was drawn,and the optimal cut-off value was determined based on the maximum Youden index,which was used as the inflection point age of the attenuation in activities of daily living among the elderly.Results1.Basic information of the elderly: among the 5570 elderly people included in this study,2745 were male,accounting for 49.28%;2825 were female,accounting for50.72%;1385 were 60-64 age groups,accounting for 24.87%;4074 were in rural areas,accounting for 73.14%;5460 were Han nationality,accounting for 98.03%;1701 were illiterate elderly,accounting for 30.54%;4848 had no religious belief,accounting for 87.04%;4246 had a spouse,accounting for 76.23%;4145 had jobs before retirement,accounting for 74.42%;496 lived alone,accounting for 8.90%;103were uninsured,accounting for 1.85%;3108 earned 1000 yuan or less per month,accounting for 55.80%.2.Current situation of activities of daily living among the elderly: The impaired rate of activities of daily living among the elderly in Henan province was 20.45%.The item with the highest damage rate was walking up and down stairs,with a damage rate of 13.97%.3.The Inflection point of decay of activities of daily living of the elderly: The inflection point age of the decay in activities of daily living among the elderly in Henan Province was 74.5 years old.The results of the Hosmer-Lemeshow test and the area under the ROC curve AUC indicated that the prediction model had good calibration and differentiation ability.4.Factors influencing the activities of daily living of the elderly: The results of univariate analysis showed that there was a statistically significant difference in the impairment rate of activities of daily living among the elderly in different age,gender,marital status,educational level,the types of health insurance,retirement career,living conditions,religion,economic income,disability,dementia,mental disease,the number of chronic illnesses,fall,lost,choking,suicidal behavior,mental state and social participation ability(P<0.05).The results of multivariate analysis showed that female,above inflection point age,living with children,disability,dementia,a higher number of chronic disease,the history of falling in the past month,impaired mental state,impaired social participation were the risk factors for activities of daily living of the elderly in Henan Province.ConclusionsGender,age,residential status,disability,dementia,the number of chronic diseases,falls,mental state and social participation were the influencing factors of activities of daily living among the elderly in Henan Province.The inflection point age of the attenuation in activities of daily living among the elderly in Henan Province was 74.5 years old,age had a high predictive value for activities of daily living.The government,society and other relevant departments should focus on the elderly at the inflection point age,and at the same time actively take corresponding measures against risk factors to gradually improve the activities of daily living of the elderly. |
