Objective:To investigate the status of benefit finding in patients with type 2 diabetes mellitus(T2DM),analyze the relationships among illness perception, social support, coping style and benefit finding, and to explore the mediating role of coping style between illness perception and benefit finding and between social support and benefit finding,so as to provide reference for the development of follow-up benefit discovery interventions. Methods:From March 2021 to October 2021,252 T2DM patients treated in the endocrinology department of a grade A hospital in Jinan,Shandong province, were selected by convenient sampling. The data were collected with the benefit-finding scale, the brief illness perception questionnaire,Perceived social support scale,Medical coping modes questionnair and self-designed general data questionnaire.SPSS 22.0 was used for data entry, collation and analysis, including descriptive statistical analysis,t-test,variance analysis,Pearson correlation analysis, multivariate linear regression. The mediation effect was tested by the Bootstrap program in the Process plugin.Results:1.Status of benefit finding, illness perception, social support and coping style in T2DM patientsA total of 252 T2DM patients were included in this study. The benefit finding score of T2DM patients ranged from 25 to 70 points, with an average of(43.45±9.77)points,which was in the middle level. The total score of illness perception was(37.34±9.17),and the average score of emotional dimension, cognitive dimension and understanding dimension were(5.03±2.08),(4.66±1.41)and(3.96±2.18)respectively.The total score of perceived social support was(63.12±9.90),the highest score of perceived family support was(22.33±3.29),and the lowest score of perceived friend support was(19.33±4.63).The scores of confrontation, avoidance and yield were(19.27±3.65),(14.60±2.84)and(8.08±2.82)respectively.The average score of each dimension from high to low was confrontation dimension(2.41±0.46),avoidance dimension(2.09±0.40),yield dimension(1.61±0.56).2.Comparison of demographic and disease characteristics of benefit finding in T2DM patientsThe benefit finding score of patients had statistical significance in age, education level, occupation, average monthly family income, types of insurance, diabetes complications and the number of complicated other chronic diseases(P<0.05).There was no significant difference in gender, religious belief, place of residence, living style and course of disease(P>0.05).3.Correlation analysis of benefit finding, illness perception, social support and coping style in T2DM patientsCorrelation analysis showed that benefit finding was significantly negatively correlated with illness perception(r=-0.251,P<0.01),significantly positively correlated with social support(r=0.417,P<0.01)and confrontation(r=0.440,P<0.01),but not significantly correlated with avoidance and yield(P>0.05).Illness perception was negatively correlated with confrontation(r=-0.165,P<0.01),and positively correlated with avoidance(r=0.214,P<0.01)and yield(r=0.543,P<0.01).Social support was positively correlated with confrontation(r=0.439,P<0.01),but not with avoidance and yield(P>0.05).4.The mediating role of coping style between illness perception and benefit finding in T2DM patientsThe results of multivariate linear regression analysis showed that illness perception was a significant negative predictor of benefit finding(β=-0.205,P=0.002)(step 1);Illness perception had a significant negative predictive effect on coping style(β=-0.151,P<0.05)(step 2);After the addition of coping style in the third step, the absolute value of regression coefficient between illness perception and benefit finding decreased(β=-0.148,P<0.05),and there was still a significant correlation between coping style and benefit finding(β=0.375,P<0.001),suggesting that coping style played a partial mediating role between illness perception and benefit finding.Further analysis found that the indirect effect of illness perception on benefit finding through the mediating effect of coping style was-0.057(-0.151*0.375),and the mediating effect was 27.8%(-0.057/-0.205).The results of Bootstrap mediating effect test showed that the Bootstrap 95%confidence interval of coping style was[-0.134,-0.016],excluding 0,indicating significant mediating effect.5.The mediating role of coping style between social support and benefit finding in T2DM patientsThe results showed that social support was a significant positive predictor of benefit finding(β=-0.205,P=0.002)(step 1);Social support positively predicted coping style(β=0.426,P<0.001)(step 2).After the addition of coping style in the third step, the regression coefficient of social support to benefit finding decreased(β=0.228,P<0.001),and there was still a significant correlation between coping style and benefit finding(β=0.298,P<0.001),suggesting that coping style played a partial mediating role between social support and benefit finding.Further analysis found that the indirect effect of social support on benefit finding through the mediating effect of coping style was 0.127(0.426*0.298),and the mediating effect was 35.8%(0.127/0.355).The results of Bootstrap mediating effect test showed that the Bootstrap 95%confidence interval of coping style was[0.078,0.205],excluding 0,indicating significant mediating effect.Conclusions:1.The level of benefit finding in T2DM patients is at a moderate level and still needs to be improved.Clinical workers should pay attention to and formulate corresponding intervention measures to improve the level of benefit finding.2.Age,education level, occupation, average monthly family income, types of insurance, diabetes complications and the number of complicated other chronic diseases were influential factors for the benefit finding of T2DM patients.3.There was a significant correlation between patients’ illness perception and benefit finding. The higher the patients’ negative knowledge of the disease, the lower the level of benefit finding, and coping style partially mediated the relationship between illness perception and benefit finding.4.Patients’ social support was positively correlated with benefit finding. The more patients perceived social support, the higher the level of benefit finding, and coping style played a partially mediating role in the relationship between social support and benefit finding.
