| Objective: Under the background of aging population,from the perspective of traditional Chinese medicine health maintenance,based on the thought of preventing disease,to understand the status quo and their relationship of traditional Chinese medicine health care literacy,psychological pain and frailty in elderly diabetes patients,and to explore the influence of traditional Chinese medicine health care literacy and psychological pain on their frailty.It will provide basic data for constructing and carrying out nursing intervention of integrated Traditional Chinese and Western medicine in elderly patients with diabetes in the future and improving the frailty of elderly patients with diabetes.Methods: A cross-sectional survey in the study of the experimental method,through convenience sampling,in February 2021 to may an endocrinologist in third rate synthetic hospital in W city in XX province and wound repair in elderly patients with diabetes to carry out research;Research tools such as general data questionnaire,TCM health care literacy questionnaire for the elderly,diabetes pain Scale and Chinese Version Tilburg weakness Scale were used to carry out the investigation.SPSS26.0 software was used for data analysis.Counting data were described by frequency and composition ratio,and measurement data were expressed by mean and standard deviation.The data satisfying normal distribution were analyzed by independent sample T test,ANOVA test,Pearson correlation analysis and multiple linear regression analysis.The test level α=0.05.Results:(1)In elderly patients with diabetes overall situation: investigating 260 people,including 60 ～74 years old elderly accounted for 69.2%,75 years old and above the elderly accounted for 30.8%;There were 131 males(50.4%)and 129 females(49.6%);There are 72(27.7%)with primary school education,92(35.4%)with junior high school education,59(22.7%)with senior high school/technical secondary school education,and 37(14.2%)with junior college education or above.(2)In elderly patients with diabetes status levels all variables: weak scores for(7.79±3.92)points,in the medium level,weak detection rate of 68.8%,Chinese medicine health care quality score for(58.16±16.43)points,with traditional Chinese medicine health care quality accounted for23.1%,higher than the overall level of our citizens.The total score of psychological pain was(3.07±1.04),which was at the serious level,and the patients with moderate or above psychological pain were the majority,accounting for 83.8%.(3)The single factor analysis showed that: in elderly patients with diabetes mellitus decline in age,culture level,marital status,nationality,personal income,health insurance payment,place of residence,such as drinking and smoking status differences statistically significant(P< 0.05).(4)Pearson correlation analysis showed that in elderly patients with diabetes in traditional Chinese medicine health care quality and psychological distress has significant negative correlation(r =-0.758,P < 0.01);There was a significant positive correlation between mental distress and frailty(r=0.715,P < 0.01).There was a significant negative correlation between health literacy of TCM and fattening(R =-0.706,P< 0.01).(5)the multivariate linear regression analysis showed that age,BMI,rural,night sleep time,merge diabetes complications,insulin therapy,Chinese medicine health care quality and the influence factors of psychological distress is weak,weak above factors were explained 65.1% of the total variation(F = 61.166,P< 0.001).Conclusion:(1)In elderly patients with diabetes medicine health literacy in the medium level,better than the overall level of China’s citizens,but with the traditional Chinese medicine health care quality accounted for only twenty percent,also has the very big promotion space;Elderly patients with diabetes have high psychological pain and poor overall condition,which should be improved continuously in the future.(2)In elderly patients with diabetes weakness in the medium level,nursing staff should focus on older age,lower education level,without a spouse,ethnic minorities,monthly income is low,Patients without medical insurance,living alone in rural areas,drinking and smoking habits,poor sleep quality,underweight,fasting blood glucose >7mmol/L,Hb A1 c ≥7%,long course of diabetes,complications and other chronic diseases,oral medication combined with insulin injection,and follow-up visits due to aggravation of the disease,In order to develop targeted personalized nursing programs to improve the senile diabetes patients’ frailty.(3)There was a negative correlation between traditional Chinese medicine health care literacy and psychological pain and weakness in elderly diabetes patients,that is,the lower the literacy level,the more serious the degree of psychological pain and weakness;Therefore,in the nursing practice of elderly diabetes patients,nurses should combine traditional Chinese and western medicine nursing,pay attention to improve the cognition of TCM health care knowledge of patients,improve their overall literacy level,so as to effectively improve the physical and mental health status of patients.(4)The doctor of traditional Chinese medicine health care quality,age,diabetes related psychological distress,at his own expense,to merge from the complications of diabetes,insulin therapy,rural,night sleep time factors in elderly patients with diabetes,such as weak status,including the doctor of traditional Chinese medicine keeping in good health health literacy is the main influence factors of weak in elderly patients with diabetes. |
