| Objective Through the analysis of the data of 302 patients with Postherpetic Neuralgia in the Qilu hospital,to summarize the clinical characteristics of patients,to explore the influencing factors of its pain degree,and to evaluate the efficacy of drug treatment,so as to provide evidence-based evidence for clinical practice.Methods The clinical data of patients with postherpetic neuralgia who were admitted into the Pain Department of Qilu Hospital of Shandong University from November 2018 to May 2021 were collected,such as name,gender,age,involved nerve segment of herpes zoster,premonitory symptoms of herpes zoster,pain score at admission,drug use,pain score at discharge,blood calcium value,and complications.SPSS 22.0 was used for statistical analysis.Mean±standard deviation was used for measurement data,and percentage(%)was used for enumeration data.Chi-square test was used for comparisons between groups.One-way ANOVA combined with linear regression model were used to analyze the influencing factors of postherpetic neuralgia pain degree,P<0.05 considered that the difference was statistically significant.Results 1.Postherpetic neuralgia was mainly distributed in patients over 60 years old,accounting for 82.7%of the total patients;among them,there were 137 males(45.3%)and 165 females(54.6%),and there was mainly thoracic ganglion involvement,accounting for 45.4%.There were 77 patients with premonitory pain symptoms before herpes.At admission,most patients suffered from moderate pain,accounting for 58.9%,followed by severe pain(99 patients,32.8%).2.After drug treatment during hospitalization,the pain score of 302 patients decreased by 3.00±1.47 points on average,and most patients were mild pain at discharge,accounting for 67.2%,with an average efficacy of 0.51±0.23.During drug treatment,the maximum oral dose of pregabalin was 150mg bid(149 patients,49.3%).Among them,227 patients(75.2%)used oxycodone and acetaminophen,and 57 patients(18.9%)used antidepressants.3.The pain degree of postherpetic neuralgia had no obvious relationship with gender,age and premonitory pain before herpes,but it was related to the involved ganglion and blood calcium value;most of patients with diabetes mellitus complicated with postherpetic neuralgia suffered from severe pain.During the treatment period,the number of people who used oxycodone,acetaminophen and antidepressants was more than that of patients without diabetes mellitus.Conclusion 1.Among the patients with postherpetic neuralgia in trigeminal nerve/facial nerve segment,the proportion of patients with severe pain is larger.2.The blood calcium value is related to the pain degree of patients with postherpetic neuralgia,and the decrease of serum calcium ion can result in the aggravation of pain.3.Pregabalin combined with oxycodone,acetaminophen and antidepressants can effectively relieve the pain symptoms of postherpetic neuralgia.The maximum dose of pregabalin is related to the pain score at admission.The higher the pain score at admission is,the greater the maximum dose of pregabalin is during treatment.4.Patients with diabetes mellitus complicated with postherpetic neuralgia account for a larger proportion of moderate and severe pain,and drug treatment can effectively relieve pain symptoms.However,compared with patients with postherpetic neuralgia,it is often necessary to combine oxycodone and acetaminophen with antidepressants to achieve better efficacy. |
