The treatment of traumatic optic neuropathy with traditional Chinese medicine is based on syndrome differentiation under the guidance of the basic theory of traditional Chinese medicine.Doctors get a clear diagnosis and put forward a treatment plan by virtue of the identification of syndromes.In fact,both ancient and modern doctors have different understanding of the etiology,pathogenesis and syndrome differentiation of traumatic optic neuropathy and their medication principles are biased,resulting in more TCM syndrome classification of traumatic optic neuropathy.Furthermore,the clinical syndrome differentiation shows diversity and there is a lack of systematic TCM syndrome dif-ferentiation to guide clinical research and treatment.Through the methods of data mining and factor analysis,this paper summarizes the medication law and syndrome characteristics of patients with traumatic optic neuropathy,which will help to give full play to the advantages of traditional Chinese medicine in the treatment of traumatic optic neuropathy and provide the basis for its clinical standardized diagnosis and treatment.Objective:1.Objectively summarize the TCM Syndrome Types and medication laws of patients with traumatic optic neuropathy,so as to provide evidence-based medical evidence for the standardization of TCM clinical syndrome differentiation and treatment of traumatic optic neuropathy.2.Using the method of factor analysis to explore the TCM syndrome of patients with traumatic optic neuropathy and summarize the TCM syndrome types of traumatic optic neuropathy,in order to provide basis for clinical syndrome differentiation.Methods:1.Explore the TCM syndrome and medication law of traumatic optic neuropathy based on data mining:The standard of Napai was formulated in advance.Through the combination of subject words and free words,the computer searched the literature on the treatment of traumatic optic neuropathy with traditional Chinese medicine from the database establishment of CNKI,Wanfang and Chongqing VIP to December 2020 and the literature was screened in strict accordance with the criteria.Spss20.0 statistical software for statistical analysis of data.The TCM syndromes,medication frequency,four qi and five flavors,drug meridians and common traditional Chinese medicine cluster analysis involved in the literature were extracted.2.TCM syndrome analysis of patients with traumatic optic neuropathy:Patients in the department of Neuro-ophthalmology,Eye Hospital,Chinese Academy of Chinese Medical Sciences from January 2020 to December 2021 were selected and the cases were reviewed by questionnaire.Taking the study of TCM syndromes as the core,collect the TCM symptoms and signs of patients with internal and external traumatic optic neuropathy in a fixed period of time.Use spss20.0 statistical software to make statistics on the gender,age and pathogenic inducement distribution of patients.Through the method of factor analysis,the TCM syndrome types of traumatic optic neuropathy were summarized by combining TCM diagnostics with clinical professional skills.Results:1.Explore the TCM syndrome and medication law of traumatic optic neuropathy based on data mining:Through the search,23 literatures meet the criteria,including 28 prescriptions and 62 traditional Chinese medicines;the core of TCM syndrome differentiation of traumatic optic neuropathy is qi stagnation and blood stasis syndrome,involving qi stagnation,blood stasis,liver deficiency and liver stagnation.qi deficiency,blood deficiency,kidney deficiency 7 syndrome elements;the top 3 Chinese medicine use frequency from high to low are angelica,chuanxiong,safflower,the medicinal flavors are mostly sweet,acrid and bitter and the medicinal properties are cold,warm and flat.According to the cluster analysis,the drugs can be grouped into four categories:the first category includes red peony root,Shengdi,citrus aurantium,achyranthes,safflower and peach kernel;the second category includes Astragalus,Shichangpu,Salvia;the third category includes Poria,Shudi,Licorice;the fourth category includes Chuanxiong,Angelica,Bupleurum.2.TCM syndrome analysis of patients with traumatic optic neuropathy:A total of 122 patients were included,including 99 males and 23 females.The age distribution was dominated by adolescents,with a median age of 18.5 years.The most common cause was traffic accident(45.1%),followed by falls(19.7%)and falls from heights(13.9%).A total of 8 common factors and their related symptoms were extracted,including F1:limb numbness,dark tongue,stringy pulse;F2:red tongue,thin pulse;F3:upset,depressed,insomnia,bitter mouth,thin yellow coating;F4:vision Unclear;F5:sore waist and knees,frequent urination at night;F6:upset,depressed,dreamy;F7:less moss;F8:not wanting to eat.After summarizing and analyzing 8 public factors,three types of syndromes were obtained:qi stagnation and blood stasis,liver stagnation and spleen deficiency,and kidney qi deficiency.Conclusion:1.The traditional Chinese medicine syndrome of traumatic optic neuropathy is characterized by qi stagnation and blood stasis throughout;the traditional Chinese medicine treatment of traumatic optic neuropathy is based on regulating qi,promoting blood circulation and removing blood stasis,focusing on regulating qi and nourishing blood and nourishing liver and kidney.2.This study objectively summarized three types of TCM syndromes in patients with traumatic optic neuropathy by factor analysis,namely qi stagnation and blood stasis syndrome,liver stagnation and spleen deficiency syndrome and kidney qi deficiency syndrome.The medication rules are consistent,which once again confirms the TCM syndrome differentiation of traumatic optic nerve degeneration and provides a reference for clinical treatment. |
