ObjectiveIn this study,the FOCUS-PDCA mode was used to improve the quality of digestive endoscope cleaning and disinfection.The qualified rate of endoscope cleaning and disinfection was improved by adjusting personnel allocation,increasing disinfection facility management,standardizing endoscope cleaning and disinfection process,strengthening personnel training,establishing supervision system and other measures.Methods1.Carry out professional training for 38 cleaning and disinfection staff in the endoscope center through FOCUS-PDCA mode,and evaluate the cleaning and disinfection knowledge,skill mastery,post ability and work attitude of cleaning and disinfection staff before and after implementation;2.480 post use digestive endoscopes were selected to evaluate the cleaning and disinfection effect of digestive endoscopes before and after the implementation of FOCUS-PDCA mode by visual observation,protein residue detection,ATP fluorescence detection technology and bacterial culture method;3.Before and after the implementation of FOCUS-PDCA mode,bacterial culture method was used to detect the bacterial content of endoscopic equipment room,disinfectant and disinfection water,and evaluate the management effect.Results.1.Knowledge,belief and practice questionnaire,the total scores of the knowledge questionnaire for 38 cleaning and disinfection staffs before and after the implementation were(25.08±1.34)and(33.97±1.28),respectively(P<0.05).The scores of the structure of flexible endoscopes,the layout and equipment of endoscopy department,cleaning and disinfection process of endoscopes,requirements and selection of water used for endoscopes,characteristics and use of endoscope disinfectants,pre-treatment of endoscopes,critical time of cleaning and disinfection,and monitor of endoscope cleaning and disinfection were higher than those before the implementation(P<0.05).For skill assessment,the skill assessment scores of cleaning and disinfection staffs before and after the implementation were(62.61±4.93)and(74.08±4.20),respectively(P<0.05).The skill assessment scores in terms of pre-treatment,leak-checking,cleaning,rinsing,disinfection,final rinsing and drying,and storage were significantly higher than those before the implementation(P<0.05).For work attitude,the attitude scores of cleaning and disinfection staffs before and after the implementation were(26.50±3.12)and(33.97±2.39),respectively(P<0.05).The scores of work attitude,learning attitude and scientific attitude of cleaning and disinfection staffs after the implementation were were significantly higher than those before the implementation(P<0.05).For post ability,the scores of the post ability questionnaire for cleaning and disinfection staffs before and after the implementation were(29.13±4.52)and(40.61±5.54),respectively(P<0.05).The scores of standard cognition,professional skill application ability,and problem-solving ability after the implementation were significantly higher than those before the implementation(P<0.05).2.For the cleaning and disinfection effect of endoscopes,the results of visual observation showed that the overall unqualified rates of endoscopes in the observation group and the control group were 2.1%and 5.4%(P=0.055).The results of protein residue detection showed that the overall unqualified rates of endoscopes in the observation group and the control group were 6.7%and18.3%(P<0.001).The results of ATP fluorescence detection showed that the overall unqualified rates of endoscopes in the observation group and control group were 5.8%and 18.3%(P<0.001).The results of bacterial culture showed that the overall unqualified rates of endoscopes in the observation group and control group were 1.7%and 10.4%(P<0.001).3.Detection of endoscope disinfection room and water showed that the unqualified rate of endoscope cleaning and disinfection room,and the unqualified rate of disinfection water after the implementation were significantly lower than those before the implementation(P<0.05).ConclusionThe application of FOCUS-PDCA model in quality improvement of endoscope cleaning and disinfection can improve the quality of endoscope cleaning and disinfection,and the quality of endoscope cleaning and disinfection staffs,with good application effect.
