Purpose:Optical coherence tomography(OCT)is used to measure the Bruch’s membrane opening-minimal rim width(BMO-MRW)in in patients with early glaucoma.Discuss the application and value of BMO-MRW in the early screening and diagnosis of glaucoma(Glaucoma)as measured by OCT.Methods:We collected 85 patients(124 eyes)diagnosed with early glaucoma from December 2019 to December 2021 in Inner Mongolia Forestry General Hospital as the test group,the test group was divided into the preclinical group: 30 patients(30 eyes);preperimetric glaucoma group: 21 patients(31 eyes);Early glaucoma group: 18 patients(32 eyes);ocular hypertension(OHT)group: 16 patients(31 eyes).Twenty-five healthy persons(50 eyes)were selected as a healthy control group for outpatient health examination.The above test group and the control group were used OCT to measure the thickness of the retinal nerve fiber layer(RNFLT)and BMO-MRW,we calculate the data information above the two groups,and then we are able to compared these data information.BMO-MRW of the four test groups were compared with healthy controls for statistical analysis of data,and the above four patients were followed for 1 year,BMO-MRW is measured and statistical analysis of data information is performed.Results :There were no statistically significant differences in the number of participants,the number of eyes,age,sex,and BCVA between the control group and test group.The comparison of RNFL in different directions between the test group and the control group: the subtemporal quadrants、supratemporal quadrants、temporal quadrants、nasal quadrants,supranasal quadrants of the experimental group were significantly lower than those of the control group,and the difference was statistically significant.Comparison of the BMO-MRW’s measurement between the test group and the control group: the BMO-MRW of the test group was significantly lower than that of the control group,the difference was statistically significant.Comparison of BMO-MRW between test groups(preclinical,OHT group,prefield glaucoma group,early glaucoma group)and control group: There was no significant difference between the preclinical and OHT groups with the control group,but the BMO-MRW between preperimetric glaucoma group and early glaucoma group was significantly lower than control group,the difference was statistically significant.Comparison of BMO-MRW indexes in preperimetric glaucoma group,early glaucoma group,OHT group and preclinical group during 1-year follow-up: The BMO-MRW of the preclinical glaucoma group and the early glaucoma group at 1-year follow-up was significantly lower than the measured value at initial diagnosis,and the difference was statistically significant.The BMO-MRW of the preclinical glaucoma group and the OHT group at 1-year follow-up at initial diagnosis showed no significant difference.In order to evaluate the ensitivity of each positive index for early glaucoma:The ROC curve results showed that the area under the ROC curve of BMO-MRW with early glaucoma diagnosis was 0.93,with a sensitivity and specificity of 96.0% and88.2%,respectively,with good predictive value.The areas under ROC curve of subtemporal quadrant,temporal quadrant and superior temporal quadrant for early diagnosis of glaucoma were all over 0.75,showing good predictive value.Conclusion:1.BMO-MRW(subtemporal quadrants)can be a sensitive indicator for the early screening of glaucoma,providing ophthalmologists with an early screening indicator for the diagnosis of glaucoma.2.BMO-MRW is less specific and sensitive than RNFLT in monitoring glaucoma progression,but it can also be used as a measurement index for monitoring glaucoma progression.
