Purpose:The application of huanglian-4 decoction after emergency PCI in patients with AMI affected troponin(CK-MB),myoglobin(cTnT),creatine kinase isoenzyme(CK-MB)and serum inflammatory factors(IL-6,TNF-α)To study the protective effect on myocardial reperfusion injury;method:Sixty patients were haphazardly divided into two groups with 30 patients in each group.The control group or experimental group was given preoperative medication(300mg aspirin,180 mg tigrelol or 300 mg clopidogrel,20 mg atorvastatin calcium),the stent was placed in the blood vessels related to myocardial infarction,and 8000 UL of heparin sodium was injected into the diseased blood vessels during the operation.Tirofiban 5 ml / h(0.2 mg / h)was injected intravenously 48 hours after operation.On the basis of conventional treatment,the trial group took 3 G Huanglian 4 Decoction twice a day for 10 days.Result One case fell off in the treatment group and one case in the control group.The efficacy and safety of 29 experimental groups and 29 control groups were compared.The results are as follows:1.There was no significant difference in past history,personal history,vital signs and the distribution of criminal vessels between the two groups(P > 0.05).2.After the test variance was used,the peak value,peak time and peak time of cTnT,MB and CK-MB in the experimental group were compared with those in the control group.After t-test,they were all greater than 0.05,that is,there was significant difference in peak time between the experimental group and the control group(P < 0.05).3.Compared with IL-6,the results of variance test before treatment were consistent between the two groups,P = 0.140 > 0.05,and there was no statistical difference.The comparison of IL-6 after treatment showed that P was close to zero,< 0.05,and the difference was statistically significant.The variance test showed the same variance before and after treatment.The results showed that P = 0.0006 < 0.05,the difference was statistically significant;4.Compared with TNF-a,the two groups showed the same difference in the test of variance before treatment,P = 0.480 > 0.05,and there was no statistical difference.Compared with TNF-a after treatment,P = 0.331 > 0.05,the difference was not significant.Results: compared with before treatment,the results showed that the variance was homogeneous,P = 0.970 > 0.05.The difference was not statistically significant.5.The safety index of the two groups before and after treatment was tested,and the p value was more than 0.05.Therefore,there was no significant difference between the experimental group and the control group(P > 0.05).6.The major cardiovascular events of mace in both groups were tested by chi square test(P > 0.05).T-test was performed for myocardial reperfusion injury,P > 0.05.Therefore,there was no significant difference between the two groups in mace and reperfusion injury(P > 0.05).Conclusion:After PCI,adding Huanglian 4 Decoction on the basis of routine treatment can improve myocardial microcirculation and reduce the damage of cardiac reperfusion.This treatment method is clinically safe,and there is no risk of myocardial reperfusion injury and other cardiovascular diseases.The mechanism is to adjust cardiac markers such as cTnT,MB and CK-MB and control inflammatory cytokines IL-6 and TNF-α It is helpful to reduce or inhibit the development of AMI,improve its prognosis,and effectively prevent the damage of ischemia-reperfusion to cardiomyocytes.
