Clinical Observation Of Gynecological Yangxue Granules In The Treatment Of Menostaxis And Metrostaxis Caused By Previous Cesarean Scar Defect

ObjectiveThe clinical efficacy and safety of gynecological yangxue granules in the treatment of menostaxis and metrostaxis caused by previous cesarean scar defect with qi-blood deficiency were analyzed,so as to provide evidence-based medicine evidence for its clinical application.MethodsIn this study,a prospective,non-randomized self-before-after observational study was used.From July 2017 to December 2019,40 patients who met the inclusion criteria in the outpatient department of gynecology of guangdong hospital of traditional Chinese medicine were selected.They started the treatment of gynecological yangxue granules orally(provided by hunan anbang pharmaceutical co.,LTD.,batch no.:national drug approval:B2002000,specification:8 bags/box,8g/bag)on the 5th day of menstruation,2 times/day,1 bag/time,for 2 weeks,for a total of 3 menstrual cycles.After drug withdrawal,patients were followed up for 3 months.The changes in menstrual period,TCM syndrome scores,the area of previous cesarean scar defect under b-ultrasound and the safety index before and after treatment and during follow-up were observed.Paired sample t test and paired symbol rank sum test were used for statistical analysis.Results1.In the improvement of menstrual period,the menstrual periods before treatment,after treatment and at the third month of follow-up were respectively(13.11 ± 2.70)d,(9.63 ± 2.91)d,(9.97 ± 2.86)d.The total effective rate after treatment was 88.57%,and the total effective rate at the third month of follow-up was 71.42%.Before and after treatment.there was significant difference in menstrual period(P<0.01).The difference in menstrual period before treatment and at the third month of follow-up was also statistically significant(P<0.01).There was no statistical difference in menstrual period after the treatment and at the third month of follow-up(P>0.05).There was no statistical difference in the curative effect of menstrual period after the treatment and at the third month of follow-up(P>0.05).The results indicated that the gynecological yangxue granules could significantly shorten the menstrual period of menostaxis and metrostaxis caused by previous cesarean scar defect,and the effect was stable.2.In the improvement of TCM syndromes,the TCM syndrome scores before treatment,after treatment and at the third month of follow-up were respectively(27.37 ± 6.87),(10.91 ± 4.53),(10.46 ± 5.47).The total effective rate was up to 100%after treatment,and the total effective rate at the third month of follow-up was 94.29%.Before and after treatment,there was significant difference in TCM symptom scores(P<0.01).The difference in TCM symptom scores before treatment and at the third month of follow-up was also statistically significant(P<0.01).There was no statistical difference in TCM symptom scores after the treatment and at the third month of follow-up(P>0.05).There was no statistical difference in the curative effect of TCM symptom scores after the treatment and at the third month of follow-up(P>0.05).The results indicated that the gynecological yangxue granules could significantly improve the TCM syndromes of menostaxis and metrostaxis with qi-blood deficiency caused by previous cesarean scar defect,and the effect was stable.3.In the improvement of the area of previous cesarean scar defect,before and after treatment,the area of previous cesarean scar defect was respectively(41.46 ± 34.05)mm2 and(36.41 ± 34.23)mm2,the total effective rate was 38.24%,and the difference before and after treatment was not statistically significant(P>0.05),which could not indicate that the area of previous cesarean scar defect after treatment was different from pre-treatment.4.There were no significant abnormalities in the safety indexes Before and after the treatment in the patients who completed the test.In the clinical trial,3 patients developed pharyngeal pain and itching during medication,but the symptoms disappeared after drinking more water.ConclusionGynecological yangxue granules can significantly shorten the menstrual period of the menostaxis and metrostaxis caused by previous cesarean scar defect,and the curative effect is stable.It can significantly improve the TCM syndromes of qi-blood deficiency,and the curative effect is also stable.However,the area of previous cesarean scar defect could not be significantly reduced.Before and after the treatment,the safety indexes(blood routine,urine routine,stool routine,coagulation function,liver and kidney function,electrocardiogram)showed no obvious abnormalities.This drug is worthy of expanding the clinical sample size and carrying out further clinical randomized controlled trials.
