The Clinical Study On The Distribution,pathology And Surgical Methods Of Pulmonary Nodules

Objective:To investigate the distribution of multiple pulmonary nodules impact on theoperation,pathology of multiple pulmonary nodules.Methods:We included 115 patients with pulmonary lung nodules by VATS operation during 2016-2017 in our hospital.The clinical data,surgical methods(including pulmonary wedge resection,pulmonary segmentectomy,lobectomy)were collected.Results:1.In this study,25 cases of patients with nodules showed the same distribution,mainly involving the right apical,right upper lobe,left on the tip section,the upper left lobe,left upper lobe and right lower lobe,right middle and upper lateral segment of the right lower lobe,medial basal segment;11 cases of patients with nodules in the same period of different leaf distribution;26 patients showed unilateral nodules of different leaf distribution;53 cases of nodules were distributed bilaterally.2.In the same period of nodule apical right distribution type(80.95%),,the upper left lobe of the lowest(10%);in the same period of different leaf nodule distribution type right apical-posterior segment of the highest rate of malignant(75%),the left anterior medial segment(62.50%),right upper medial basal segment-before the end of the lowest(00.00%);in the nodules of different leaf types of unilateral distribution,left upper lobe-lower lobe malignant rate was the highest(71.43%),followed by the mid-right upper lobe(60%),right upper lobe-lower lobe was the lowest(33.33%malignant).3.Wedge resection was performed in 54 cases,segmental resection in 6 cases,lobectomy in 47 cases,lobectomy combined with wedge resection in 7 cases,and bilateral operation in 1 cases.4.This group cases of postoperative complications in lung infection rate was the highest,up to 3.48%;4 cases of lobectomy arrhythmia;2 cases of pulmonary lobectomy combined with wedge resection leakage;1 cases of wedge resection cases of pulmonary infarction;1 cases lobectomy appears bronchopleural fistula;1 cases lobectomy of subcutaneous emphysema.Conclusion:Multiple pulmonary nodules in the lungs of the different distribution and pathology of benign and malignant,choose different operation methods;multiple pulmonary nodules in lung cancer,metastatic tumor,tuberculosis,fibrosis nodules,inflammatory pseudotumor,intrapulmonary lymph nodes and other common multiple pulmonary nodules;overall survival rate is high,the recent quality of life better recurrence and metastasis is low,should be treated actively.
