| Objective In this study,we investigated the presence of preoperative anxiety in patients undergoing elective surgery in the Department of orthopedics.Whilst analyzed the clinical data on the level of preoperative anxiety.We wanted to provide a basis for effective intervention.The method of acupoint stimulation was used to intervene patients with anxiety before elective surgery in the Department of orthopedics.The effect of acupoint stimulation on preoperative anxiety,sleep quality as well as objective index of blood pressure and pulse were explored.In addition,we discussed the correlation between preoperative anxiety and sleep quality.Methods We used the Hamilton anxiety scale to evaluate the anxiety level of our patients undergoing elective surgery in the Department of orthopedics.And screened patients with anxiety.82 patients who met the inclusion and exclusion criteria and were selected as subjects.They were randomly divided into test and control groups,(41 patients in each group).In the control group,the routine nursing in the Department of orthopedics was carried out,including the observation of the disease,the improvement of preoperative examination,the diet nursing,the guidance of functional exercise,and health education and psychological nursing.The test group implemented the acupoint stimulation,the exposure blocking therapy in behavior therapy and the percutaneous electric stimulation of the bilateral Neiguan point.This took place between the day of admission to 1 days before the operation.The daily number of intervention is less than 1,the total number of intervention is more than 3.The evaluation indicates the patient’s anxiety level,sleep status,blood pressure and pulse rate.The anxiety level was assessed by the Hamilton anxiety scale and the anxiety visual analogue scale,and the sleep status was assessed by the St.Mary Hospital Sleep Questionnaire.The above were evaluated at the time of admission and operation day’s morning.Comparing the difference of anxiety level,sleep status,blood pressure and pulse between the two groups at admission and on the morning of operation.By doing this we evaluated the effectiveness of acupoint stimulation.Results 1.In this study,280 cases of elective surgery diagnosed as fractures were screened,274 cases were investigated effectively,of which 127 patients had preoperative anxiety,82 patients were eligible for inclusion criteria,and the incidence of preoperative anxiety was 46.3%.2.Anxiety level:there was no significant difference in the HAMA and A-VAS scores between the two groups before intervention(P>0.05).After intervention,there was a significant difference in the HAMA and A-VAS scores between the two groups(P<0.05).The experimental group intervention HAMA score declined,patients accounted for 92.68%,mainly in the decline of 3-6,The control groups declined,patients accounted for 17.07%,a decline of-1~-2 HAMA score in patients with increased 60.98%.The experimental group intervention A-VAS score declined,87.80%of the patients,mainly in the decline of 1~2.The control groups declined,patients accounted for 19.51%,a decline of-1~-2.It’s A-VAS score in patients increased 53.66%.3.Sleep status:there was no significant difference in SMH scores between the two groups before intervention(P>0.05).But there was significant difference between the two groups of SMH scores after intervention(P<0.05).There was a correlation between the HAMA score and the SMH score in the two groups(the correlation coefficient was 0.647,P<0.05).4.Blood pressure and pulse rate:there was no significant difference in blood pressure and pulse rate between the two groups before intervention(P>0.05).However after intervention,there was a significant difference in pulse rate between the two groups(P<0.05),but there was no significant difference in blood pressure(P>0.05).Conclusion 1.The incidence of preoperative anxiety in the patients who are undergoing elective surgery in the Department of orthopedics is high,and clinical attention should be paid to this problem.2.To a certain degree,preoperative anxiety affects the patient’s sleeping condition.3.As a psychological method,acupoint stimulation can reduce the preoperative anxiety level,and improve the sleep status of the patient while regulating preoperative anxiety.4.Further research is needed to confirm the effect of acupoint stimulation on blood pressure and pulse. |
