Arthroscopic Reduction And Internal Fixation Of Schatzker Ⅱ-Ⅳ Tibial Plateau Fracture With The Assistance Of A Precise Reduce Device And Bone Graft Kits

Objective:to examine a newly designed precise reduce device and bone graft kits in the application of arthroscopic treatment of tibial plateau fracture;and to evaluate their clinical effect compared with the traditional ORIF.Methods Design of the devices1)Reformative design of the cruciate ligament locator:we add an additional horizontal carved arm to the cruciate ligament locator.By doing this,the fractured fragment and be fixed by another K wire from the horizontal arm and collapse of the fragment can be avoid theoretically;2)We design a blot with a hole in the core and scale,then the ring saw can be used as a bone graft device under the guide of K wire and the blot.With the accompany of a "Z" shape pry stick,the bone graft and be implanted more precisely.2.clinical data30 patients with SchatzkerⅡ—Ⅳ fractures in our department from 2016.06 to 2017.06 were included.Patients were older than 65 years.Patients were randomly divided into ORIF and ARIF(arthroscopic reduction and internal fication).Operation time,length of incision,bleeding,volume of bone graft,complications and HSS grade 3,6 and 12 months after operation were compared between the two groups.Resluts There was no significant difference(SD,p=0.069)in operation time betweenthe two groups,although the ARIF group seems to needs a relatively long time.Compared with the ARIF group,the ARIF group have a significant less bleeding,shorter incision,less bone graft volume and better HSS grade 3 month after operation(p<0.05).No SD on HSS grade 6,12 months after operation between the groups.Conclusions Arthroscopic reduction and internal fixation of Schatzker Ⅱ-Ⅳ tibial plateau fracture with the assistance of a precise reduce device and bone graft kits is a good method compared with the traditional ORIF method...
