Objective To analyze the differences in the related risk factors of cerebral small vessel disease in Lhasa-Beijing area,analyze the factors affecting the severity of white matter lesions and lacunar infarction in Lhasa area,and analyze whether there are differences in the image spatial distribution of patients with cerebral small vessel disease in the two regions.Whether high altitude factors in plateau areas have a unique impact on cerebral small vessel disease provides evidence for the early prevention of the disease in plateau areas.Methods Under the premise of meeting the medical ethics requirements and obtaining the informed consent of the research subjects,according to the relevant standards(inclusion criteria and exclusion criteria)formulated in this study,they were retrospectively included in the patients who attended the People’s Hospital of Tibet Autonomous Region from January 2017 to December 2019.169 patients with cerebrovascular disease belong to the Lhasa group,and 200 patients with cerebrovascular disease who were in the Department of Neurology of Peking University First Hospital belong to the Beijing group.Review their clinical data,summarize and summarize clinical data such as current medical history,personal history,past history,etc.,To evaluate the spatial distribution characteristics of the patient’s MRI.Compare the two groups’ age,gender,chronic disease history,blood lipids,serum homocysteine(homocysteine,Hcy)and other indicators,analyze the differences in risk factors between the two places;use head MRI to assess the severity of white matter lesions and luminal infarction in patients.Through univariate analysis of the factors affecting the severity of white matter lesions and lacunar infarction,the spatial differences in the distribution of white matter lesions and lacunar infarctions in imaging lesions in the two places were analyzed.Statistical method: use SPSS 21.0statistical software for statistical analysis.The measurement data conforming to the normal distribution are expressed as x ± s,and the t test is used for comparison between groups.The non-normally distributed measurement data are represented by the median,and the rank sum test is used for comparison between groups.Categorical variables are represented by percentiles,and the comparison is performed by χ 2 test.P <0.05 is considered statistically significant.Results There are more men with cerebral small vascular disease in Beijing,and more patients with small vascular disease in Beijing are diagnosed with hypertension,diabetes,hyperlipidemia,and coronary heart disease.More patients with small vascular disease in Beijing have Smoking history and family history of cerebrovascular disease.More patients with cerebral small vessel disease in Lhasa have a history of drinking.Compared with patients in the Lhasa area,the incidence of white matter lesions is higher and more serious,and the incidence of lacunar infarction is lower but more severe.White matter lesions in patients with high altitude are more distributed in the frontal lobe and basal ganglia,LI is more distributed in the cerebellum,and less in the frontal lobe and hippocampus.Conclusion The severity of white matter lesions in plateau areas is closely related to advanced age and hypertension,and the severity of lacunar infarction is related to age,hypertension,diabetes,and smoking.Compared with plain areas,white matter disease changes are more likely to occur and the severity is more serious in plateau areas.The degree of sexual infarction is also more serious.Patients in high altitude areas need to control blood sugar and blood pressure more strictly,reduce smoking,and avoid the aggravation of cerebral small vessel disease.
