Effects Of Braces Combined With BSPTS Therapy On Moderate Adolescent Idiopathic Scoliosis

Background:At present,brace-wearing is the main method for the treatment of moderate adolescent idiopathic scoliosis,which can effectively prevent the aggravation of scoliosis,while the impact of the brace on the quality of patients' life cannot be ignored.The efficacy of Physiotherapeutic Scoliosis-Specific Exercises(PSSE)on adolescent idiopathic scoliosis has also been widely verified.The Society on Scoliosis Orthopaedic and Rehabilitation Treatment(SOSORT)proposes to combine PSSE and the brace to improve the efficacy of correction and reduce the complications of brace-wearing,but there are few related studies.Aims:To explore the efficacy of combination of brace-wearing and BSPTS therapy on moderate adolescent idiopathic scoliosis;to analyze the role of BSPTS therapy in brace treatment.Methods:The subjects of the study include 40 patients with moderate AIS in the Rehabilitation Department of Tianjin Hospital.The subjects were randomly divided into two groups,20 cases in the experimental group and 20 cases in the control group.With the intervention time of 24 weeks,the experimental group was treated with brace-wearing and BSPTS therapy,while the control group was treated with the brace only.All subjects were observed the changes in Cobb angle,angle of Trunk Rotation(ATR),vertebral rotation Raimond,CAL,thoracic kyphosis(TK)and SRS-22 questionnaire before and after the treatment.Results:1.The Cobb angle of the main curve averagely decreased by 9.72° after the treatment in the experimental group,while decreased by 4.77° in the control group.There was no significant difference between the two groups before treatment(P>0.05),but a significant difference between the two groups after treatment(P<0.05).Efficacy evaluation on the improvement rate of the Cobb angle of the main curve of the two groups after the intervention showed that the improvement rate of the experimental group was 72%,the stability rate was 27%,and the deterioration rate was 0%,while the control group was 44%,50% and 6%.2.The ATR in the experimental group decreased by 3.77 ° averagely after the intervention with a significant difference(P<0.05),and the ATR decreased by 1.22°on average after the intervention in the control group with no significant difference(P>0.05).3.The Raimondi value in the experimental group decreased by 9.61°on average after the intervention with a significant difference(P<0.05),while in the control group,the Raimondi value averagely decreased by 3.89°after the treatment with no significant difference(P<0.05).4.The experimental group reduced CAL by 6.92 mm after the treatment with a significant difference(P<0.05).The control group had an average reduction of 3.41 mm after the treatment with no significant difference(P>0.05).5.The TK value of the experimental group declined by 3.61 ± 4.45 ° after the treatment with no significant difference(P>0.05),while the TK value of the control group decreased by 9.20°after the treatment,with a significant difference(P<0.05).The comparison between the flat back deformity rate of the two groups of patients before and after intervention indicated that the flat back deformity rate of the control group was 38.9%,while that of the experimental group was 16.7%.Compared with the control group,the flat back deformity rate of the experimental group decreased by22.2%.6.There was no significant difference in pain before and after the treatment in the experimental group(P>0.05).The total score of SRS-22,self-image,functional activity,and mental health decreased,with a significant difference(P<0.05).In the control group,the total score of SRS-22,pain,self-image,mental health,and functional activity declined after the treatment,with significant difference(P<0.05).The total score of SRS-22,pain,self-image,mental health,and functional activity had no significant different between the two groups before the treatment(P>0.05).After the treatment,the pain score of the control group was significantly lower than that of the experimental group,with a significant difference(P<0.05)),and there was no significant difference in the total score of SRS-22,self-image,mental health,and functional activity(P<0.05).Conclusions:1.BSPTS therapy can remarkably strengthen the efficacy of the brace and improve the patient's Cobb angle.The combination of the two has better effect than wearing the brace alone.2.During the period of wearing the brace,BSPTS therapy can better improve the patient's ATR,vertebral rotation Raimond and CAL,and improve the patient's Razor back deformity and posture imbalance.3.The combination of BSPTS therapy and the brace can effectively prevent flat back caused by the brace.4.The quality of patients' life during brace treatment decreases.BSPTS therapy can relieve the pain caused by the brace and improve treatment satisfaction.
