Objective:To investigate the mechanism of the disease Objective to investigate the uptake mechanism of 99mTc-6-hydrazinylnicotinyl octreotide(HTOC)in non tumor tissues and to develop clinical examination methods.Method:Objective To retrospectively analyze the image characteristics and clinical data of 6099 cases of somatostatin receptor imaging in the Department of nuclear medicine,Peking Union Medical College Hospital,Chinese Academy of Medical Sciences from January 2017 to January 2020.The images were interpreted by two reporting physicians engaged in nuclear medicine.The regions of interest were delineated by planar and fusion imaging,and the ratio of target tissue to non target tissue was recorded.The physiological uptake was analyzed by vision,and all non-specific uptake was analyzed by semi quantitative data.Result:1.In pituitary non tumor uptake and tumor uptake,P = 0.02<0.05;Non tumor uptake and tumor uptake of adrenal gland,P=0.002<0.05;Non tumor uptake and tumor uptake in pancreatic tissue,P=0.002<0.05.The uptake of tumor imaging agents was significantly higher than that of non tumor imaging agents.2.Point uptake in lung.In fusion imaging,the target/background ratio of point uptake in lung was 6.96-28.30,with an average of 12.80.Visual analysis grading planar imaging and fusion imaging were grade 2 in 5 cases.The uptake of radioactivity in the lung was increased in a patchy and diffuse manner.The visual grade of planar imaging was grade 2,and the intensity of radioactivity uptake was low.3.Localized uptake of intestinal tract was found in 10 cases,12 fingers in 7 cases,ileocecal part in 2 cases and presacral intestine in 1 case.The images showed localized high uptake of imaging agent,and some of them showed segmental diffuse uptake.The visual grading was 2-3.The ratio of intestine to muscle was 2.65-20.44.4.There was significant difference in breast tissue uptake between women of childbearing age and menopause(P=0.019<0.05).There was no significant difference between the two cycles of breast tissue in childbearing age(P>0.05).5.There was a significant difference in uterine uptake between menopause and childbearing age(P=0.004<0.05).There were significant statistical differences among menstrual period,proliferative period and secretory period,and there were significant statistical differences among three weeks,P=0.042<0.05.The average value of endometrial radioactive uptake in proliferative period was the lowest.There were significant differences in endometrial uptake between late secretory phase and proliferative phase(P=0.030),late secretory phase and ovulation phase(P=0.007);There was no significant difference between menstrual and late secretory(P=0.387),proliferative and menstrual(P=0.151),proliferative and ovulatory(P=0.549).The endometrial uptake in menstrual period is higher than that in secretory period,but not in ovulation period.Conclusion 99mTc-6-hydrazinylnicotinyl[tyr3]-octreotide(HTOC)can be found in multiple sites of non neoplastic uptake.In pituitary,adrenal and pancreatic tissues,the target/background ratio of neuroendocrine tumor uptake imaging agent is higher than that of non neoplastic uptake.The non-specific uptake of imaging agents by these organs may be related to their functions.The uptake of imaging agent by uterus and mammary gland of menopausal women is lower than that of reproductive women.Women's physiological cycle will affect the uptake of imaging agent by mammary gland and uterus.Proliferative period may be the best time for women to check.For the non-specific uptake in the lung,it can be caused by embolic blood vessels with small emboli.For the non-specific uptake in the lung and the limited non-specific uptake in the intestine,it is mostly caused by inflammation.The gallbladder is one of the excretory organs of imaging agents,or the effect on imaging can be reduced by diet and other methods.
