| Objective:To observe the self-evaluation of various pain dimensions and the improvement of range of motion of cervical vertebra before and after treatment and 1 month follow-up,and to provide a feasible and optimal treatment plan for clinical treatment of CEH by selecting the method of tendon focal point acupuncture based on anatomical structure of neck. Methods:All 19 patients with CEH who fulfilled were studied by using the method of self-control.The treatment points of choice were nodules, cords or tenderness points in the muscularly ventral area of the target muscle.By inspection and palpation, the target muscles are musculus rectus capitis posterior major which is on the tight side, musculus obliquus capitis inferior which is on the other side, musculus splenius capitis which is on the same side that mastoid process is further back than the other, sternoceleidomastoid which is on the other side, and acromiotrapezius muscle which is on the same side that acromial end is higher.During acupuncture, the local skin at the insertion point was fixed with the hand, and a 0.25×40mm needle was quickly pricked into the skin with the needle hand. The acupuncture effect was measured by the beating of the target muscle and the reduction of the muscle tension in the hand of the doctor. The needle was performed once the effect was achieved. Each patient was treated twice a week, with an interval of 1-3 days,4 times as a course of treatment, and totally 1 course per patients. The patients were followed up before the treatment, after 2 and 4 times of treatment, and after 1 month of treatment, respectively. The simplified McGill questionnaire score and cervical motion score were used to compare the difference of curative effect before and after treatment.SPSS22.0 software was used for statistical analysis of all the obtained data. The result P<0.05 indicated that the difference between the obtained results was statistically significant;if P<0.01,that difference indication between the obtained results was statistically significant.Results:1.Comparison of the total scores of the simplified McGill questionnaire before treatment and after 2 times of treatment:the total scores of the simplified McGill questionnaire were lower than those before treatment at the middle of treatment, and the difference between the two groups was statistically significant(P<0.01),indicating that this therapy had produced significant effects on the overall situation of headache of patients with cervical headache after 2 times of treatment.2.Before and after 2 times of treatment on the simplified McGill questionnaire each dimension score comparison:mid treatment was simplified the McGill questionnaire each dimension score was reduced, and a statistically significant difference was found in each group scores(P<0.01),and this therapy in the treatment of headache in patients with cervical source sex have a headache after 2 times each dimension has significant curative effect.3.Comparison of ROM scores of cervical vertebra before treatment and after 2 times of treatment:the ROM scores of cervical vertebra in the middle treatment decreased compared with before treatment, and the difference between the two groups was statistically significant(P<0.01),indicating that therapy has produced a significant effect on the range of motion of cervical vertebra in patients with cervical headache after 2 times of treatment.4.Comparison of the total scores of the simplified McGill questionnaire before and after the treatment:the total scores of the simplified McGill questionnaire after the treatment were lower than those before the treatment, and the difference between the two groups was statistically significant(P<0.01),indicating that this therapy has a significant effect on the overall headache situation of patients with cervical headache after 4 times of treatment.5.Before and after treatment in the simplified McGill questionnaire each dimension score comparison:after the treatment was simplified the McGill questionnaire each dimension score was reduced, and a statistically significant difference was found in each group scores(P<0.01),and this therapy in the treatment of headache in patients with cervical source sex have a headache after 4 times each dimension has significant curative effect.6.Comparison of ROM scores of cervical vertebra before and after the treatment:after treatment,ROM scores of cervical vertebra decreased compared with before treatment, and the difference between the two groups was statistically significant(P<0.01),indicating that this therapy has a significant effect on the range of motion of cervical vertebra in patients with cervical headache after 4 times of treatment.7.Evaluation of curative effect before and after treatment:after 4 times of treatment, the total effective rate of the simplified McGill questionnaire reached 94.7%,and the healing significance rate reached 89.5%.8.Evaluation of follow-up:compared with the four times after treatment, the treatment of end of a month when simplified McGill questionnaire of sensory dimension, emotional dimension,PPI scores are rising, but both no statistical significance(P>0.05),compared this therapy in the three dimensions of efficacy in the treatment when after the end of a month, but the decline is not obvious;VAS score and ROM score of cervical spine were the same as those after 4 times of treatment(P>0.05),indicating that the efficacy of this therapy in pain degree and range of motion of cervical spine could last for at least 1 month. The total score of the simplified McGill questionnaire was increased compared with that after 4 times of treatment, and the comparison between the two was statistically significant(P<0.05),indicating that in terms of the overall assessment of pain, the efficacy of this therapy decreased after 1 month of treatment.Research Conclusions:1.Selection of tendon focal point acupuncture therapy based on the neck structure is a method for the treatment of cervical headache proposed based on the theory of meridian of traditional Chinese medicine and human anatomy. This method has a significant effect on pain and neck motion of patients with cervical headache.2.This therapy has significant effects on all dimensions of patients’ self-evaluation of pain.3.After 4 times of treatment for 2 weeks, the improvement effect on the pain degree and range of motion of the patient’s cervical spine can last for at least 1 month; The improvement effect of sensory dimension, affective dimension and current pain degree of patients’ self-evaluation of pain decreased after 1 month. |
