Evaluation Value Of 3D Slicer-based Imaging Indicators For Short-term Prognosis Of Spontaneous Intracerebral Hemorrhage

Objective:The 3D Slicer was used to measure imaging metrics regarding patients with spontaneous intracerebral hemorrhage,to explore its impact on the short-term prognosis of spontaneous cerebral hemorrhage,and to screen for independent predictors.Methods:The clinical data and 3D Slicer-based imaging indices of patients diagnosed with spontaneous intracerebral hemorrhage at Changsha Central Hospital from October 2019 to October 2020 were retrospectively analysed,and the prognosis of patients at 30 days of onset was grouped according to the modified Rankin Scale(mRS)score,using univariate and multifactorial analyses to finally obtain independent predictors of short-term prognosis of patients with cerebral haemorrhage Relevant factors.Results:A total of 95 consecutive valid cases were included in t his study.54 cases in the good prognosis group(mRS<3)and 41 cases in the poor prognosis group(mRS≥3).The general clinical data of the patients were analyzed univariately,and the statistical results showed that the age of the poor prognosis group was higher than that of the good prognosis group,and the difference was statistically significant(P<0.05).The differences in clinical data such as gender,systolic blood pressure at admission,diastolic blood pressure at admission,history of smoking,history of alcohol consumption,history of diabetes mellitus and hyperlipidemia were not statistically significant(P>0.05).According to the analysis of imaging data,the differences between the good prognosis group and the poor prognosis group were statistically significant in terms of hematoma location,hematoma volume,hematoma morp-hology,hematoma density and rate of perihematomal edema expansion.Factors that may affect the prognosis of cerebral hemorrhage were included in the multifactorial logistic regression analysis,which finally resulted in independent factors affecting the prognosis of spontaneous cerebral hemorrhage including age(OR=1.070,95%CI:1.011-1.133,P=0.020),hematoma site(P=0.003),hematoma volume(OR=1.396,95%CI:1.136-1.716,p=0.002),rate of perihematomal edema expansion(OR=1.778,95%CI:1.244-2.541,P=0.002),and hematoma morphology(OR=7.337,95%C I:1.513-35.569,P=0.013),the established model was statistically signifi-cant(p<0.001),with a specificity of 84.6%,sensitivity of 85.7%,positiv e predictive value of 88.9%and negative predictive value of 80.5%.Conclusions:Imaging indicators measured based on 3D Slicer software have a certain assessment value for prognosis of spontaneous cerebral hemorrhage,and the growth rate of patient’s age,hematoma site,hematoma volume,hematoma morphology and rate of perihematomal edema expansion can be used as independent predictors of poor prognosis(mRS≥3)of patients after 30 days.
