Clinical Study On The Treatment Of Chloasma Due To Liver Stagnation And Blood Stasis With Modified Shugan Huoxue Decoction

Research background Melasma is a condition that affects the face.Occurence is higher in females than males.Although there are a variety of Chinese and Western medicine in clinical treatment.,there is still no effective treatment method or procedure.After a number of clinical observation and clinical experience combined with my instructors,we found the most common clinical type of liver depression and blood stasis.We found that many patients with liver stagnation and blood stasis made poor sleep.Which combined with Anshen in the treatment(Anshen,Shugan,promoting blood)can achieve better results.Objective The study is to observe the effect of the addition or subtraction Shugan Huoxue decoction on liver stagnation and blood stasis melasma and to investigate the mechanism that promoting blood Shugan Anshen treat liver stagnation and blood stasis melasma.As the same time it provided an objective basis for clinical application.Methods This rasearch has collected a total of 86 patients who visited the Third Affiliated Hospital of Zhejiang Medical University from December 2014 to December 2015 that complied with the diagnostic criteria.All patients were given a questionnaire to collect the basic information.All patients were given addition or subtraction Shugan Huoxue decoction twice daily.Follow-ups were conducted every 6 weeks,up to aperiod of 12 weeks.Questionnaire included:basic information,size and color of lesion,tongue,pulse and associated symptoms,quality of life(ALQI scale)and sleep(AIS scale).And from 48 patients and normal population from outpatient preventive treatment of disease each randomly selected 48 cases were detected hemorheology to contrast and observe correlation between the two.Results(1)To compare rates from skin lesions,67.39%of patients had decreased after 6 weeks89.73%of patients had decreased after 12 weeks.12 weeks of treatment was 68.75%effective.For symptom score of TCM,91.30%of patients had decreased about the lesions scores after 6 week.92.31%of patients had decreased about the lesions scores after 12 week.12 weeks of treatment was 83.33%effective.(2)By AIS scale score,63.95%of 86 patients had a diagnosis of insomnia,23.26%diagnosed as suspected insomnia.The scale score declined in traditional Chinese medicine treatment.The AIS score were significantly different after 6weeks and 12 weeks(p<0.001).(3)By ALQI scale score,73.91%of patients had decreased after 6 weeks.87.18%of patients had decreased after 12 weeks.The ALQI score were significantly different after 6weeks and 12 weeks(p<0.001).(4)48 patients randomly selected from 86 patients and normal population detected hemorrheology.The results show that high-cut,cut,low-cut,plasma viscosity,Casson viscosity and erythrocyte aggregation index difference was not significant between the two(p>0.05).(5)About melasma patients with liver stagnation and blood stasis,there is a positive correlation between course of disease and lesion.there is a positive correlation between lesion score and quality of sleep.There also is also a positive between TCM symptom scores and quality of sleep.Conclusion Modified Shugan Huoxue decoction is to treat melasma patients with liver stagnation and blood stasis have better clinical efficacy.did not find any adverse reaction.Through the treatment of melasma patients with symptoms,quality of life,quality of sleep were significantly improved.
