| Objective: To compare the clinicopathological features and prognosis of patients with different stages of left-Sided Colon Cancer(LCC)and right-Sided Colon Cancer(RCC)after radical resection,and analyze the difference between them.Therefore,a reasonable individualized treatment plan is formulated for clinical work,and a certain foundation is laid for further research on the pathogenesis of left and right colon cancer.Methods: The clinicopathological data of 476 patients undergoing radical colon cancer surgery in our Hospital from January 2011 to December 2015 were retrospectively analyzed.According to the anatomical site of tumor,patients of colon cancer were divided into left colon cancer group(LCC group,257 cases)and right colon cancer group(RCC group,219 cases).Data such as gender,age,first clinical symptoms,preoperative hemoglobin,carcinoembryonic antigen(CEA),sugar antigen(CA19-9),histopathological type,tumor size,tumor differentiation degree and postoperative TNM stage were collected and compared between the two groups by referring to cases.Telephone follow-up,outpatient review and other ways to regularly understand the patient’s 5-year overall survival rate.SPSS 25.0 was used for statistical analysis,counting data were expressed as rates,and c2 test was used for inter-group comparison.Kaplan-meier curve and log-rank test were used for survival analysis and comparison.Multivariate analyses were conducted by Cox risk proportional regression model,and P < 0.05 was considered statistically significant.Results: 1.The proportion of male in LCC group was higher than that in RCC group,and the proportion of female in RCC group was higher than that in LCC group.2. Compared with LCC group,anemia,abdominal mass,larger tumor diameter and low differentiation adenocarcinoma were predominant in RCC group.The proportion of mucinous adenocarcinoma and sigma-ring cell carcinoma was higher in RCC group.In the LCC group,blood or bowel habit change was the main clinical first symptom(P < 0.05).3.There were statistically significant differences in preoperative CEA and CA19-9 positive rates between the two groups(P < 0.05).4.The 5-year overall survival rates of LCC group and RCC group were 66.4% and 64.3%,respectively(P > 0.05).The 5-year overall survival rate was 86.9% in LCC group and 83.3% in RCC group in stageⅠcancers(P > 0.05).In stageⅡ and Ⅲ cancers,5-year overall survival rates were 65.9% and 59.7% in LCC group and 76.6% and44.2% in RCC group,respectively(P < 0.05).5.Multivariate analysis showed differentiation degree(HR=0.569,95%CI: 0.339-0.956),p T stage(HR=2.128,95%CI:1.306-3.467)and p N stage(HR=2.659,95%CI: 1.846-3.828)was an independent risk factor for 5-year survival after colon cancer surgery(P < 0.05).Conclusion: There are differences in clinicopathological features and survival prognosis between left and right colon cancer.There was no significant difference in the 5-year overall survival rate between them.The prognosis of colon cancer in different parts of the Group was stage-dependent.In stageⅠcancers,the prognosis of left and right colon cancer is similar.In stageⅡcancers,right colon cancer has a good prognosis.In stage Ⅲ cancers,right colon cancer has a worse prognosis.Low tumor differentiation and late p T and p N stage suggest low survival rate. |
