Objective:1.To investigate and analyse the clinical characteristics,skin care habits and patient awareness about photoaging of facial skin photoaging in the Dali region.2.To observe the effectiveness and safety of Periplaneta Americana extract combined with microneedling in the treatment of facial skin photoaging.Methods:1.To collect a total of 120 patients who attended the outpatient clinics of the Department of Dermatology and the Department of Medical Aesthetics of the First Affiliated Hospital of Dali University and were diagnosed with facial skin photoaging between February 2021 and February 2022.The self-developed questionnaires were completed by them,and the results were analyzed descriptively.2.60 patients with facial photoaging who met the inclusion criteria were selected for the clinical randomized controlled study,and were divided into experimental and control groups according to the random number method.They were treated with microneedle percutaneous introduction of Periplaneta Americana extract or saline,respectively,for a total of 3 treatments,each 4 weeks apart.Subjective assessments of global score for photoaging and evaluation of subjects’ pain and satisfaction were performed at different time points during the treatment period.while changes in facial wrinkles,texture,pores,spots,UV pigmentation and other indicators were objectively evaluated using the VISIA detectors.Adverse reactions and the time of their appearance and regression were recorded for all subjects during the follow-up period.Results:1.120 patients with facial skin photoaging in the Dali region had Glogau photodamage grading predominantly grade II(35.00%)and grade III(49.17%),and Fitzpatrick skin typing was most common with type IV(65.00%),mostly dry skin(35.00%)and oily skin(25.83%).Photoaging features were most obvious in pigmentation(52.50%),which appeared mainly at the age of 36-45 years(40.00%);followed by wrinkles(41.66%),which appeared mainly at the age of 46-55 years(38.33%).7.50% of the patients did not have any sun protection measures,19.17% used sunscreen skin care products,and the knowledge rate of SPF and PA was 16.67%.the dermatology life of quality index score was 0.63±1.75.2.After 3 treatments,except for the spot score,wrinkles,texture,pores,and UV pigmentation scores in the experimental group and the control group were higher than before treatment,and the Global Score for Photoaging was lower than before,and the differences were statistically significant(P < 0.05).After treatment,the experimental group had higher wrinkles,pores and UV pigmentation scores than the control group,and lower the Global Score for Photoaging than the control group,all differences were statistically significant(P < 0.05).The mean time to improve wrinkles,UV discoloration onset and the mean time to fade postoperative erythema in the experimental group were shorter than in the control group,with statistical differences(P < 0.05).The subjects in the experimental and control groups were 85.71% and 82.75% satisfied with the improvement of skin firmness,respectively,with no statistical difference(P > 0.05),and85.71% and 75.86% satisfied with the improvement of skin texture,respectively,with statistical difference(P < 0.05).No adverse reactions such as hyperpigmentation or hypopigmentation,scar formation and aggravation of melasma were observed in both the experimental and control groups.Conclusions:1.120 patients with facial skin photoaging in Dali region were mainly with grade II or III photoaging and Fitzpatrick IV skin typing,which were prevalent in dry and oily skin.The photoaging features were most prominent in pigmentation and appeared earlier than wrinkles.The impact of photoaging on patients’ quality of life is not obvious,and most patients’ perception of photoaging has problems such as lack of attention,insufficient knowledge,inadequate sun protection and skin care measures,and other influencing factors being ignored.2.Periplaneta Americana extract combined with microneedling has an improvement effect on texture roughness,coarse pores and epidermal melanin associated with photoaging and improves overall photoaging level.It also has the advantages of short postoperative erythema fading time,no adverse reactions such as pigmentation or hypopigmentation,scar formation and melasma aggravation.Therefore,it can be used as an effective measure to improve photoaging in areas with strong UV rays and in people with darker skin tone or with melasma.
