Objective To investigate the effect of alcohol consumption on plasma low-density lipoprotein cholesterol/high-density lipoprotein cholesterol ratio(LDL-C/HDL-C ratio,LHR)and intracranial artery stenosis in patients with ischemic stroke.Methods A total of 420 patients with ischemic stroke who were treated in the Department of Neurology,Fuyang People’s Hospital from January 2020 to January 2021 were selected and divided into drinking group(236 cases)and non-drinking group(184cases)according to the history of drinking..The general baseline data of the patients,plasma HDL-C,LDL-C,homocysteine(Hcy)and other hematological indicators within24 hours of the onset of the disease were collected,and the patients with ischemic stroke were subjected to etiological classification and imaging studies.The data were evaluated for the severity of intracranial artery stenosis,and the differences in the above data were compared between the two groups of patients with ischemic stroke.Binary Logistic regression was used to analyze the risk factors of alcohol consumption affecting the occurrence of ischemic stroke and the correlation between alcohol consumption and intracranial arterial stenosis.Results The proportion of males,smoking history,hypertension,diabetes,NIHSS score,LDL-C,LHR and Hcy levels on admission in the drinking group were higher than those in the non-drinking group,and the differences were statistically significant(P<0.05);Logistic regression model analysis of factors showed that gender,smoking,NIHSS score,LHR and hyperhomocysteinemia were risk factors for ischemic stroke caused by alcohol consumption(P<0.05);ROC curve analysis showed that LHR judged alcohol consumption The area under the curve of patients with ischemic stroke caused by ischemic stroke was 0.70(95% confidence interval: 0.649-0.750).When the LHR cutoff value was 2.34,the sensitivity was 72.9% and the specificity was 65.2%;Compared with the detection of amino acid alone,the combined detection of LHR and Hcy had a higher predictive value for the risk of ischemic stroke caused by alcohol consumption(AUC 0.835,95%CI: 0.796-0.874,P<0.001),and the sensitivity was92.1%.The specificity was 89.6%.The proportion of large artery atherosclerotic stroke in the drinking group was higher than that in the non-drinking group,and the number and severity of intracranial artery stenosis in the drinking group were significantly higher than those in the non-drinking group,and drinking was an independent risk factor for intracranial artery stenosis(P<0.001).Conclusion 1.The patients in the drinking group were mostly male and younger,and the NIHSS score,LDL-C,LHR and Hcy levels were significantly increased in the drinking group,and the risk of large atherosclerotic stroke was increased.2.Male,smoking,NIHSS score,LHR and Hcy were independent risk factors for ischemic stroke in the drinking group.3.Alcohol consumption,smoking,hypertension and LHR are independent risk factors for intracranial arterial stenosis,and the higher the LHR level,the higher the risk of intracranial arterial stenosis,which is a better biomarker for predicting the development of atherosclerosis.
