| Objective:To investigate the effects of different doses of iodixanol and age on early renal function in patients after PCI in high altitude areas.Methods:A total of 89 patients who underwent coronary angiography and pereutaneous coronary intervention(PCI)for the diagnosis and treatment of coronary stenosis in Hospital between September 2020 and September 2021 were selected.Divided into group A(< 200 m L)and group B(≥200 m L)according to the dose of contrast agent used during PCI.The research subjects were collected including: age,sex,height,weight,history of chronic diseases,history of renal insufficiency,biochemical indicators within24 hours after admission,serum creatinine(Scr)before surgery and 48~72 hours after surgery.),cystatin C(Cys C),urea,uric acid(UA),intraoperative dose of iodixanol,and postoperative use of ACEI/ARB and other drugs.The calculation of the endogenous creatinine clearance rate(Ccr)is based on the Cockcroft-Gault formula: Ccr=[(140-age)×weight(kg)]/[0.818×Scr(μmol/L)],if it is a woman,the calculation result× 0.85.SPSS 20.0 software was used to analyze the effect of different doses of iodixanol on the early renal function of patients of different ages after PCI in middle and high altitude areas.Results:There were 10 cases of contrast-induced acute kidney injury(CI-AKI)in group A(<200 m L group)and group B(≥200 m L group),of which 5 cases in group A,The incidence rate was 6.94%;5 cases occurred in group B,and the incidence rate was29.41%.There was a statistically significant difference between group A and group B,P=0.027).It shows that the higher the dose of contrast agent,the higher the incidence of CI-AKI.The increase of Scr in group B was more obvious than that in group A at 48-72 hours after operation,and the difference between the two groups was statistically significant(P<0.05).Comparing group A1(age <70 years old)and A2 group(age ≥70years old),there was no significant difference in renal function before and after surgery(P>0.05);comparing group B1(age <70 years old)and B2 There was no significant difference in renal function between the groups(age ≥70 years old)before and after surgery(P>0.05).Conclusions:1.For patients undergoing elective PCI in middle and high altitude areas,the dose of isotonic contrast agent iodixanol has a significant effect on the renal function of patients,and the higher the dose,the greater the probability of renal function damage,and the incidence of contrast agent acute kidney injury.higher rate.2.The results of the study show that the use of iodixanol for elective PCI in patients of different ages is safe and feasible,and there is no significant change in renal function due to the increase of age. |
