Objective: Worldwide increasing prevalence of obesity and lifestyle changes has put nonalcoholic fatty liver disease(NAFLD)as the most prevalent liver disease of the coming decade.NAFLD not only causes cirrhosis and hepatocellular carcinoma,but also acts as a component of metabolic syndrome together with obesity,type 2 diabetes mellitus,cardiovascular disease,and dyslipidemia.Consequently,its mutual cause-and-effect interactions have brought a huge clinical and financial burden to patients and society.Non-alcoholic fatty liver disease(NAFLD)has a higher incidence in obese children and a greater harm,which needs to cause people’s attention.In this study,the risk factors of non-alcoholic fatty liver in obese children by analyzing relevant clinical indicators of obese children.Methods: In this study,a retrostective study was used to collect the clinical data of children diagnosed as NAFLD admitted to department of pediatric of The Second Affiliated Hospital of Anhui Medical University from January 2017 to January 2021.102 patients were finally involved.Recording height,weight,blood biochemistry indexes and Liver ultrasound results,and calculate The body mass index and insulin resistance index.Then the children were divided into NAFLD groups and non-NAFLD groups according to the diagnostic criteria of non-alcoholic fatty liver disease of chidren.The risk factors of NAFLD were analyzed by single factor and multivariate logistic analysis.Results:1.Compared with the non-NAFLD group,sex ratio,body mass index of the NAFLD group(BMI),waist circumference(WC),systolic blood pressure(SBP),insulin resistance index(HOMA-IR),triglyceride(TG),aspartate(AST),alanine aminotransferase ALT)and uric acid(UA)were significantly statistical difference.However,there was no statistical difference between age,diastolic blood pressure(DBP),total cholesterol(TC),high-density lipoprotein cholesterol(HDL-C),and low-density lipoprotein cholesterol(LDL-C).2.The indicators related to non-alcoholic fatty liver disease were selected as the independent variables,and NAFLD was included as the dependent variable in the logistic multivariate regression analysis.The results showed that ALT,SBP,UA and WC were the independent risk factors for NAFLD in simple obese children.3.The ROC curve analysis of NAFLD in ALT,SBP,UA and WC showed that the area under the curves of ALT,SBP,UA,WC was 0.829,0.657,0.801,0.756 respectively.UA and WC has high sensitivity and specificity in the prognostication of NAFLD in obese child,and ALT has high specificity of prognostication that,SBP having high sensitivity.UA and WC can be considered a sensitivity and specificity index of prognostication of NAFLD,ALT is specific index of that,and SBP is a sensitive index.Conclusions :Elevated ALT,SBP and UA are independent risk factors for NAFLD in obese children,and monitoring relevant indicators has important reference value for the assessment of disease.These findings can serve as a signal for the early identification of NAFLD in obese children,and are important for early prevention and treatment to prevent progression to irreversible serious disease.
