Objective To explore the relationship among childhood trauma and obsessive-compulsive disorder(OCD)symptoms and implicit memory(IM)and explicit memory(EM).Obsessive-compulsive disorder(OCD)is a common clinical mental disorder,which will bring great pain and burden to patients and their families.At present,drug therapy and cognitive therapy are the main treatment methods.Studies have shown that the more traumatic childhood experiences,the more likely you are to develop psychopathological symptoms,including obsessive-compulsive symptoms.Foreign studies have shown that early exposure to trauma is associated with poor social cognition and working memory deficits.The study of Yousefi et al.pointed out that the memory function of patients with Obsessive-compulsive disorder was defective.Some scholars believe that children who are abused continue to be affected by traumatic experiences that persist in the brain in the form of traumatic memories,making them vulnerable to mental illness,including obsessive-compulsive disorder.At present,there are many studies on childhood trauma and memory of OCD patients abroad,but they mainly focus on working memory,while domestic studies are relatively rare.Therefore,this study investigated the relationship among childhood traumatic experience and obsessive-compulsive disorder(OCD)symptoms as well as implicit and explicit memory by questionnaire measurement,word perception speed test and word recognition test.Methods According to International Classification of Diseases and Related Health Problems,Tenth Revision(ICD-10),59 patients diagnosed with obsessive-compulsive disorder were included,and recruited 41 healthy people matched with gender and age as normal controls.The childhood trauma was investigated by using the childhood trauma questionnaire short-form(CTQ-SF).The accuracy and response time during vocabulary perception speed test tasks and vocabulary recognition tasks were recorded to index implicit memory and explicit memory performance.The obsessive-compulsive symptoms were assessed with Yale-Brown obsessive-compulsive scale(Y-BOCS).Results Compared with the control group,the scores of emotional abuse(9.48±3.21 for OCD group,6.46±2.41 for controls group,t =5.092,P<0.01),physical abuse(7.39±2.91 for OCD group,5.31 ± 0.85 for controls group,t = 3.329,P<0.01),sexual abuse(6.25±2.51 for OCD group,5.07±0.26 for controls group,t =2.990,P<0.01),emotional neglect(11.49±3.88 for OCD group,8.53±2.71 for controls group,t =4.204,P<0.01),physical neglect(8.58±2.81 for OCD group,7.24±2.24 for controls group,t=2.525,P<0.05)and overall childhood trauma(43.19±10.58 for OCD group,32.64±6.11 for controls group,t =5.748,P<0.01)were different in the obsessive-compulsive disorder group.There are differences in implicit memory accuracy.The total score of OCD(22.48±6.25)was positively correlated with the total score of childhood trauma(r=0.292,P<0.05).The scores of obsessions symptoms(12.52 ± 4.61)were positively correlated with the total scores of CTQ-SF(P<0.05).There was no correlation between compulsive behavior and childhood trauma score.There was a negative correlation between the scores of physical abuse and the accuracy of explicit memory(69.69±13.11,r=-0.266,P<0.05).The scores of physical neglect was positively correlated with implicit memory response time(3821.32±2190.50,r=-0.281,P<0.05).The scores of physical neglect was negatively correlated with the accuracy of implicit memory(51.41±31.14,r=-0.369,P<0.01).Conclusion Compared with healthy people,obsessive-compulsive disorder patients had more traumatic experiences in childhood,and their implicit memory was obviously impaired,while their explicit memory was not abnormal.Obsessions in patients with OCD are related to traumatic childhood experiences,especially physical abuse and sexual abuse.Childhood physical trauma may affect the level of implicit and explicit memory in patients with obsessive-compulsive disorder.
