| Research Purpose:1.Deeply analyze the demand for health education for hospitalized COPD patients in different periods of diagnosis,treatment and rehabilitation,the ways of obtaining health education,the opinions and suggestions on hospital-led health education,and how to strengthen the compliance of health education for COPD patients.2.Establish COPD patient health education knowledge base.3.Based on this knowledge base,the construction of a health education path for COPD patients in a clinical decision support system is completed.Research Methods:1.Based on literature review and ERG theory,qualitative research method was used to conduct semi-structured interviews with COPD patients and medical staff in the respiratory medicine department,to deeply analyze the needs of hospitalized COPD patients for health education in different periods such as diagnosis,treatment and rehabilitation,and their preference for receiving health education methods,and to explore how to strengthen the compliance of health education for COPD patients.Based on literature review and qualitative research results,the knowledge base of health education for COPD patients was preliminarily established.2.The Delphi expert consultation method was used to demonstrate and modify the items of the preliminarily drawn-up knowledge base of health education for COPD patients.When the opinions of the experts who inquired about the information tend to be consistent,the research team combined the feedback of experts to determine the final items of the knowledge base of health education for COPD patients,and construct the path of health education for COPD patients based on the clinical decision support system.3.The knowledge base of COPD patients’ health education is embedded in the system,and the three modules of data extraction,intelligent decision making,and automatic push are used to complete the construction of COPD patients’ health education path.Research Results:1.Through semi-structured interviews with COPD patients,the following seven topics were obtained:(1)the demand of COPD patients for disease-related knowledge at the admission stage;(2)The demand for knowledge related to various auxiliary examinations;(3)Demand for drug-related knowledge;(4)The demand for knowledge related to disease self-management in stable period;(4)psychosocial support needs;(6)Diversified means of acquiring health education knowledge;(7)Views and suggestions on hospital-led health education.2.Based on the semi-structured interviews with medical staff in the respiratory department,five themes were obtained:(1)The health education needs of hospitalized patients with acute exacerbation COPD were focused on symptom distress and disease outcome;(2)COPD patients lack the knowledge of acute attack prevention,monitoring,and emergency management;(3)Health education needs of stable COPD patients focus on symptom management,respiratory rehabilitation,and other aspects;(4)The health education needs of drugs and diet nutrition in COPD patients run through the whole process of diagnosis,treatment,and nursing;(5)Suggestions on strengthening COPD patients’ compliance with health education.3.After two rounds of Delphi expert consultation,the COPD patient health education knowledge was finally determined,including 3 first-level items,16 second-level items,and 45third-level items.The recovery rates of the two rounds of expert correspondence questionnaires were 94.12% and 100%,respectively.The authority coefficient(Cr)of the first round and the second round was 0.891 and 0.913 respectively.The coefficient of variation(CV)of items at all levels in the two rounds of expert consultation was all < 0.3.In the first round of expert letter consultation,the Kendall coordination coefficient of items at level 1,level 2 and level 3 were0.292(P < 0.009),0.416(P < 0.001)and 0.376(P < 0.001),respectively.In the second round of expert consultation,the Kendall coordination coefficients of the first,second and third-level items were 0.292(P < 0.001),0.436(P < 0.001)and 0.361(P < 0.001),respectively.4.With COPD patients’ health education knowledge base as the knowledge source,the path of COPD health education in hospital health education clinical decision support system was constructed by using three modules of health education clinical decision support system: data extraction,intelligent decision making,and automatic push of health education content.Research Conclusion:1.There are different demands for health education in hospitalized COPD patients at different stages of disease,diagnosis and treatment.Medical staff should provide continuous and personalized health education for patients,and actively explore new health education approaches dominated by hospitals to continuously improve the effectiveness and quality of health education for patients.2.Combined with literature review and qualitative research results,the knowledge base of health education for COPD patients was preliminarily formulated.The Delphi expert consultation method was used to establish the final knowledge base of health education for COPD patients,which covers the health education contents of COPD patients in different stages of diagnosis,treatment and rehabilitation,and provides material sources and guidance for health education for COPD patients.3.The construction of health education path for COPD patients based on clinical decision support system makes the health education of COPD patients have obvious flexibility.It is no longer a single health education path for COPD diseases,but a health education path for different individuals of COPD patients,which is conducive to the realization of personalized and accurate health education for COPD patients. |
