Research Purpose:1.Providing a convenient emotional tool of experience assessment for patients who cannot communicate verbally in a special environment of the ICU;2.Paying attention to the mental health of ICU patients.Carrying out experimental research on emotional experience.Exploring the applicability and effect of affective pictures and SAM in ICU patients,so as to provide reference for promoting positive emotional experience of ICU patients.Research Methods:This study was an experimental study.From January 2021 to December 2021,patients in the comprehensive ICU of a third tertiary hospital in Hangzhou were selected.The research objects were selected strictly according to the inclusion and exclusion criteria,grouped and sorted according to the random number table method,and then divided into group 1 and group 2.Within3 days,group 1 was given the stimulation of six positive emotional pictures and the evaluation of emotional experience every day.And group 2 was given the stimulation of six neutral emotional pictures and the evaluation of emotional experience every day.Both groups received routine psychological care.SPSS24.0 was used to analyze the data.The mean ± standard deviation was used to describe the scores of each dimension of different emotional picture experience.And t-test was used to compare the scores of each dimension of emotional picture experience and the comparison of each group with the norm in two groups at different time points.And drawing the time change trend chart of each dimension of emotional picture experience.Two-dimensional spatial distribution scatter plot was used to analyze the correlation of scatter plot of each dimension of emotion.After the experiment,the ICU nurses’ usage feelings and the patients’ feelings about emotional pictures were collected.Research Results:1.Baseline data about two groups of subjectsThe baseline comparative analysis of gender,age,admission diagnosis,marital status,education level,occupation,religious belief,medical payment method,monthly family income,smoking and drinking history,average hospital stay,other chronic diseases,drug use,the number of pipelines,the use of mechanical ventilation,the use of protective restraint,the number of family visits,APACHE II score,pleasure score,arousal score,dominance score and so on between the two groups showed that the difference was no statistically significant(P>0.05).2.The experience scores of affective picture about the two groups of research subjects(1)Pleasure score(1)Comparison between groups: The results showed that there was no statistically significant difference between the two groups in the pleasure score of emotional experience on the first day,except for SAM3(P>0.05).There was significant differences on statistic between two groups in emotional scores experience pleasure on the 2nd and 3rd days(P<0.05).(2)Comparison with the norm value: The results showed that the overall pleasure of the two groups of subjects was lower than the norm.Among them,the comparison of the pictures of SAM1,SAM3,SAM5,SAM6 and the norm on the first day of the group 1 was statistically significant(P<0.05).The comparison of SAM7,SAM9,SAM11,SAM12 and the norm on the second day of group 1 was statistically significant(P<0.05).And the comparison of SAM14,SAM16,SAM17,SAM18 and the norm on the third day of group 1 was statistically significant(P<0.05).The comparison of the pictures of SAM1,SAM2,SAM3,SAM5 and the norm on the first day of the experimental group 2.The comparison of SAM7,SAM9,SAM11,SAM12 and the norm on the second day of group 2 was statistically significant(P<0.05).And the comparison of SAM13,SAM15,SAM17,SAM18 and the norm on the third day of group 2 was statistically significant(P<0.05).(3)Longitudinal comparison: The pleasure score of group 1 showed a continuous upward trend with time.And the fluctuation range of the pleasure score of group 2 with time was small,almost parallel trend.(2)Arousal score(1)Comparison between groups: The results showed that there were statistically significant differences in SAM3 and SAM5 between the two groups in the arousal score of emotional experience on the first day(P<0.05).The difference in SAM11 on the second day was statistically significant(P<0.05).And the difference in SAM15 and SAM17 on the third day was statistically significant(P<0.05).(2)Comparison with the norm value: The results showed that the arousal degree of the two groups of subjects was lower than the norm as a whole.Among them,the comparison of the pictures of SAM1,SAM3,SAM5,SAM6 and the norm on the first day of group 1,the difference was statistically significant(P<0.05).And the comparison of the pictures of SAM7,SAM9,SAM11 and the norm on the second day of group 1,the difference was statistically significant(P<0.05).And the comparison of the pictures of SAM13,SAM15,SAM17,SAM18 and the norm on the second day of group 1,the difference was statistically significant(P<0.05).The comparison of the pictures of SAM1,SAM3,SAM5,SAM6 and the norm on the first day of the group 2showed that the difference was statistically significant(P<0.05).The comparison of the pictures of SAM8,SAM9,SAM10,SAM12 and the norm on the second day of the group 2 showed that the difference was statistically significant(P<0.05).The comparison of the pictures of SAM13,SAM15,SAM17,SAM18 and the norm on the third day of the group 2 showed that the difference was statistically significant(P<0.05).(3)Longitudinal comparison: The arousal scores of the two groups of research subjects showed a trend of increasing at first and then decreasing with time.(3)dominance score(1)Comparison between groups: The results showed that there was no significant difference in dominance scores of affective experience between the two groups on the first day and the second day(P>0.05).On the third day,there were statistically significant differences in dominance score of emotional experience(P<0.05).(2)Comparison with the norm value: The results show that the overall dominance of the two groups of subjects is lower than the norm.Among them,the comparison of the pictures of SAM1,SAM3,SAM5 and the norm on the first day of the group 1,the difference was statistically significant(P<0.05).And the comparison of the pictures of SAM7,SAM11 and the norm on the second day of group 1,the difference was statistically significant(P<0.05).The difference was statistically significant when comparing the pictures of SAM14,SAM16,SAM17,SAM18 and the norm on the third day(P<0.05).The comparison of all affective pictures and norm in experimental group 2 on the first day showed a statistically significant difference(P<0.05).And the comparison of SAM7,SAM10,SAM11 pictures and norm on the second day showed a statistically significant difference(P<0.05).The SAM16,SAM17 pictures and norm comparison on the third day,the difference was statistically significant(P<0.05).(3)Longitudinal comparison: The dominance score of group 1 showed a gradual upward trend with time,and the dominance score of group 2 fluctuated within a small range over time,almost parallel.(4)Scatter plot of various dimensions of emotionFrom the scatter plots of the three dimension scores of ICU patients on affective pictures of different dimensions and levels,a correlation between the three dimensions can be seen.(1)Experimental group 1: The degree of pleasure increased continuously over time,and gradually maintained at a medium and high level.And the degree of arousal was distributed from scattered high and low levels to concentrated at low and medium levels.Pleasure and dominance gradually became oblique.Arousal gradually maintained at a moderate level.(2)Experimental group 2: The degree of pleasure and dominance gradually became oblique,and the correlation effects of other dimensions had no obvious regularity.3.The feedback on usage and experience(1)The feelings of the implementer: The scores of the ICU nurses’ feelings about usage from questionnaire showed that the scores of each item were above 4.00.The total score was21.00±1.00.And the overall agreement rate was 100%;(2)The feelings of the receiver: ICU patients have a high overall acceptance of emotional picture experience.Patients generally expressed that the evaluation method was easy to understand.They were interested in pictures and satisfied with this emotional picture experience.They were willing to continue to accept emotional picture experience.Research Conclusion:1.ICU patients can generate positive emotions through affective pictures experience,and the emotional effect induced by affective pictures is better than that of neutral pictures.High-level dimensional pictures are not easy to reach the level of normal people,but low-level dimensional pictures can reach or even higher than normal people’s reactions.However,the level of emotional experience induced by high level dimensional pictures is still higher than that of low level dimensional pictures.It is recommended to use high-level dimensional pictures to stimulate emotional experience.Especially after 2 days of conscious awareness,the effect of affective picture experience begins to be obvious.2.There is a correlation between the three dimensions of emotion.The selection of affective pictures should not only consider the valence of pictures,but also pay attention to other dimensions of emotion.It is recommended to use pictures of high pleasure and high arousal level to improve the degree of pleasure.And using pictures of high arousal and high dominance to increase the degree of arousal.And to increase the degree of dominance,it is recommended to use pictures of high dominance level.3.Emotion pictures and SAM scale have good applicability,high acceptance and high clinical feasibility in ICU patients.And it can be used as an assessment tool for emotional experience of ICU patients,especially for those who cannot communicate verbally.
