| Objective:The aim of this study was to explore the relationship between various indicators and late-onset eclampsia by analyzing the blood lipid levels(TC,TG,LDL-C,HDL-C,Apo A1,Apo B),uric acid, and cystatin C in patients with different parity late-onset preeclampsia.previous relationship. In order to assist clinicians in evaluating late-onset preeclampsia, intervene early, delay pregnancy termination, and significantly improve the prognosis of mothers and infants. Methods:The clinical data of 354 patients with late-onset preeclampsia and normal pregnant women without other complications in our hospital from October 2019 to October2021 were analyzed retrospectively.According to the number of deliveries, the parturients were divided into two groups:normal pregnancy group(n=126),preeclampsia group(n=60)and severe preeclampsia group(n=112).There were 112 cases in severe preeclampsia group(59 cases in primary parturient and 53 cases in multipartum). The differences of basic data(age, parity,BMI,blood pressure)and clinical indexes(blood lipids(TC,TG,HDL-C,LDL-C,Apo A1,Apo B,LDL-C/HDL-C,Apo A1/Apo B),uric acid and cystatin C)among the three groups with different parity were analyzed, and a multivariate regression model was established to explore the correlation between each index and late-onset PE.Results:1.Among primiparas, the levels of TC,TG,LDL-C,LDL-C/HDL-C,uric acid, and cystatin C in the severe PE group were higher than those in the normal pregnancy group and the PE group, and the difference was statistically significant(P<0.05),and showed that the severe PE group>the PE group>the normal pregnancy group;there was no significant difference in the levels of HDL-C,Apo A1,Apo B,and Apo A1/Apo B among the three groups(P>0.05).2.Compared with normal pregnant women, the levels of TC,TG,LDL-C,LDL-C/HDL-C,uric acid and cystatin C in patients with preeclampsia were significantly higher in patients with preeclampsia, the highest in severe PE group, the second in PE group and the lowest in normal pregnancy group. There was significant difference among the three groups in terms of Apo A1/Apo B level:normal pregnancy group>PE group>severe PE group. There was no significant difference in HDL-C,Apo A1 and Apo B among the three groups.3.Nine variables such as age,BMI,number of deliveries,TC,TG,LDL-C,LDL-C/HDL-C,uric acid and cystatin C were included in the binary logistic regressionmodel for multivariate analysis. It was found that BMI(24.0-27.9kg/m~2),TG(2.98-3.68mmol/L),LDL-C(>2.97mmol/L),LDL-C/HDLC(1.74-2.10)and uric acid(>356.75umol/L)were the related factors of late-onset PE.With reference to the lowest quartile, the OR values and 95%CI of BMI,TG,LDL-C,LDL-C/HDL-C and uric acid in the highest quartile were5.30(1.44-19.60),4.28(1.87-9.77),6.27(2.21-18.52),6.02(2.01-18.01),4.30(1.78-10.39),respectively.Conclusion:1.The levels of blood lipids, uric acid and cystatin C in late-onset preeclampsia were significantly higher than those in normal pregnant women, and gradually increased with the aggravation of the disease. These indicators may correlate with the severity of PE.2.BMI,TG,LDL-C,LDL-C/HDL-C,and uric acid are related factors of late-onset PE,and their dynamic changes should be monitored.3.There was no significant correlation between parity and preeclampsia in the multivariate analysis of this study, but primipara and multiparous women had common related factors, and BMI,TG,LDL-C,LDL-C/HDL-C,uric acid, cystatin There are varying degrees of changes in the level of C.In the future, more attention should be paid to the research on pregnant and lying-in women in different population structures to make the prevention and treatment work better. |
