| Objective:To summarize and analyze the clinical data(clinical manifestations,laboratory test results,pathological examination and prognosis)of colorectal cancer patients of different ages,and to understand the tendency and differences of clinical and pathological features of colorectal cancer patients of different ages,so as to improve the screening awareness of colorectal cancer and other diseases among people of different ages and similar clinical manifestations.In order to achieve the early detection and diagnosis of the corresponding diseases,improve the survival rate of related patients and reduce the mortality.Research objects and methods:Patients with colorectal cancer treated in the second Hospital of Jilin University from February 2011 to February 2019 were selected,and the corresponding clinical data were collected.including age,sex,clinical manifestations,past history,family history,laboratory test results,pathological reports and follow-up information,etc.,according to different age groups(≤45 years old group,46-65 years old group,≥66 years old group)for statistical analysis.Results:1.A total of 2132 colorectal cancer patients were finally included in this study after screening,of which,the patients’ per person age was 61.92±11.2 years,and the patients’ minimum age was 19 years and maximum age was 89 years.There were 147 colorectal cancer patients aged ≤45 years,with a male to female ratio of 1.23:1.The most common clinical symptom in this age group was bloody or melena,with five patients showing no clinical symptoms.The positive rate of fecal occult blood test was the highest among laboratory test results.There were 1175 patients with colorectal cancer in the age group 46-65 years,with a male to female ratio of 1.67:1.The most common clinical symptom of patients in this age group was bloody or melena,and 81 of these patients,who did not exhibit any clinical symptoms,developed lesions for daily physical examination.Patients in this age group mostly had no concomitant disease or previous history,and the common concomitant disease was hypertension.The positive rate of fecal occult blood test was the highest among laboratory tests.There were 810 colorectal cancer patients in the age≥66 years group,with a male to female ratio of 1.37:1.The most common clinical symptom of patients in this age group was bloody or black stools,and 66 patients did not have any clinical symptoms to detect lesions for daily physical examination.Common concomitant diseases and past medical history were hypertension.The positive rate of fecal occult blood test was the highest among laboratory tests.The differences in clinical symptoms among the three groups were not statistically significant.2.The most common pathological type of patients in the age group≤45 years old was adenocarcinoma,non-special type,and the most common differentiation grade was moderate differentiation.The most common stages of TNM pathology in patients of this age group are stage Ⅲ and stage Ⅳ.The most common pathological type of TNM in 46-65 years old group was adenocarcinoma,non-special type,and the most common differentiation grade was moderate differentiation.The most common pathological stages in this age group were stage Ⅱ and Ⅲ.The most common pathological type of patients in ≥66 years old group was adenocarcinoma,non-special type,the most common grade of differentiation was moderate differentiation,and the most common stages were stage Ⅱ and stage Ⅲ.There was significant difference in staging between ≤45 years old group and 46-65 years old group,≤45 years old group and ≥ 66 years old group.Patients in ≤45 years old group were more likely to stage Ⅲ and Ⅳ tumors than 46-65 years old group and ≥66 years old group.There was no significant difference between 46-65 years old group and ≥66 years old group,indicating that there was no difference in staging between 46-65 years old group and ≥66 years old group.3.The overall survival of patients with CRC in the ≤45 years group was lower than that in the 46-65 years group and higher than that in the≥66 years group.Age,histological classification,degree of differentiation and stage are independent factors affecting the prognosis of patients with colorectal cancer.Conclusion:1.Compared with middle-aged and elderly patients with colorectal cancer,the malignant tumors of young patients with colorectal cancer have the characteristics of poor histological type and late clinical staging.There is no significant difference between young patients with colorectal cancer and middle-aged and elderly patients with colorectal cancer in terms of clinical symptoms.More attention should be paid to young people,and to young people who show nonspecific symptoms such as hematochezia and changes in defecation habits.The possibility of developing colorectal cancer should be considered and further examination should be conducted.Actively promote the disease to develop into primary prevention of colorectal cancer,popularize the concept of early diagnosis and early treatment of related diseases,and effectively improve the survival rate of the related population.2.The incidence of colorectal cancer is mainly middle-aged and elderly patients,the middle-aged and elderly people with hematochezia or changes in defecation habits should not be ignored,we should be vigilant against the possibility of colorectal cancer or rectal cancer,and seek medical treatment as soon as possible.In this study,some middle-aged and elderly patients with colorectal cancer did not show any clinical symptoms,so the screening of colorectal cancer in middle-aged and elderly people should be further promoted.3.The overall survival rate of colorectal cancer patients in the young group was lower than that in the middle-aged group,but higher than that in the old group.Age,histological classification,degree of differentiation and stage are independent factors affecting the prognosis of patients with colorectal cancer. |
