| Objective:(1)To retrospectively analyze the clinical characteristics of 243 patients with NAs treated with hepatitis B cirrhosis of low level viremia.(2)To observe whether there are differences in laboratory indicators and complications in patients with hepatitis B cirrhosis of low level viremia treated with different NAs drugs.Materials and Methods:We collected the general data and clinical data of 243 patients treated with nucleos(t)ide analogues who were admitted to the Department of Hepatobiliary and Pancreatic Medicine in our hospital from June 2018 to December 2019 and met the inclusion and exclusion criteria,and analyzed the basic conditions(sex,age,etc.),virological indicators,liver function indicators,complications,and antiviral drug treatment,etc.According to the different types of NAs used in antiviral therapy,patients were divided into four groups: entecavir group,tenofovir disoproxil fumarate tablet group,tenofovir fumarate alafenamide group and other groups.We analyzed and compared the laboratory test results(HBs Ag,HBe Ag,HBV DNA,PLT,PTA,ALT,AST,ALB,TBIL,DBIL,IBIL,etc.)and complications(ascites,hypersplenism,esophagogastric varices,esophagogastric varices bleeding,hepatic encephalopathy,primary liver cancer,portal vein thrombosis,spontaneous peritonitis,liver failure,hepatorenal syndrome).At the same time,in this study,the minimum detection limit of domestic reagents for HBV DNA quantification was 50 IU/m L,and the minimum detection limit of imported reagents was 20 IU/m L.The subjects were divided into two groups according to HBV DNA quantification: <50 IU/m L and 50-2000IU/m L.We compared the laboratory test results of the two groups of patients.At the same time,we compared the complications of NAs-treated patients with hepatitis B cirrhosis of low level viremia in different genders and different ages.Result:(1)The patients in this study were mainly male,with 165 cases(67.9%)and 78females(32.1%).Most of the patients were middle-aged and elderly patients,with an average age of(53.9±9.5)years.The 45-60 age group had the largest number(144cases,accounting for 59.2%).(2)In this study,the NAs used in antiviral therapy were mainly entecavir,with188 cases(77.37%).32 cases(13.17%)were treated with tenofovir disoproxil fumarate,18 cases(7.41%)were treated with tenofovir fumarate,3 cases(1.23%)were treated with lamivudine,and 2 cases(0.82%)were treated with adefovir dipivoxil.(3)In this study,according to HBV DNA quantification,there were 133 cases(54.7%)in the HBV DNA quantification group <50 IU/m L,and 110 cases(45.3%)in the 50-2000IU/m L group.(4)In this study,the qualitative difference of HBe Ag between the two groups with quantitative HBV DNA was statistically significant(P<0.05),and the quantitative grouping of HBV DNA and qualitative difference of HBe Ag in the four groups with different antiviral drugs were not statistically significant(P >0.05).(5)AST,ALT,TP,DBIL were statistically significant in the comparison of laboratory test indexes of the two groups of patients with HBV DNA quantification in this study(P<0.05),and the other indexes were not statistically significant(P>0.05).The comparison results of laboratory test indexes of the four groups of patients with different antiviral drugs were not statistically significant(P>0.05).(6)In this study,the incidence of primary liver cancer among the complications of the four groups of patients with different antiviral drugs was statistically significant(P<0.05),and the other complications such as hypersplenism,hepatic encephalopathy,and ascites were not statistically significant(P>0.05).(7)In this study,the complications of NAs-treated patients with hepatitis B cirrhosis of low level viremia in different genders and ages were statistically significant(P<0.05).Conclusion:(1)The incidence of NAs-treated hepatitis B cirrhosis of low level viremia is significantly higher in males than in females,mainly in the age group of 45-60 years old.(2)The qualitative difference of HBe Ag between the two groups was statistically significant(P<0.05),but there was no significant difference in the quantitative grouping of HBV DNA and the qualitative difference of HBe Ag between the four groups with different antiviral drugs(P>0.05).(3)In the comparison results of laboratory test indexes of HBV DNA quantitative patients in the two groups,the indexes with statistical significance were AST,ALT,TP,DBIL(P<0.05),and the other indexes were not statistically significant(P>0.05).(4)The comparison results of laboratory test indexes of the four groups of patients with different antiviral drugs were not statistically significant(P>0.05). |
