Research On Home-based Pulmonary Rehabilitation Model For Patients After Lung Cancer Surgery Based On WeChat Applet

Objective:To clarify the understanding and awareness of home-based pulmonary rehabilitation in patients after lung cancer surgery,and their needs and perspectives for WeChat applet supported home-based pulmonary rehabilitation;combined with relevant domestic and foreign literature,and analysis of patients’ needs for home-based pulmonary rehabilitation,constructing module content of home-based pulmonary rehabilitation model for patients after lung cancer surgery based on WeChat applet;the content of each module of the applet is pre-experimented,pulmonary rehabilitation training is carried out for patients after lung cancer surgery,and the effect is evaluated.Based on the results of the pre-experiment,the WeChat applet was further improved to provide a basis for the release and promotion of the WeChat applet for home pulmonary rehabilitation in the later stage.Methods:Part Ⅰ:Analysis of home-based pulmonary rehabilitation needs of patients after lung cancer surgeryThe method of descriptive qualitative research was used,and the objective was to select patients after lung cancer surgery for semi-structured interviews,who were hospitalized in the Department of Thoracic Surgery of the First Hospital of Jilin University from December 2020 to April 2021,in order to explore the understanding of home-based pulmonary rehabilitation of patients after lung cancer surgery,and the demand and suggestions for home-based pulmonary rehabilitation WeChat applet.The interview outline was developed based on the literature review and pre-interview results,and the interview was conducted in the patient’s ward or conference room.Interviews continued until the material was saturated and no new topics emerged.The interviews were recorded with the consent of the respondents and immediately transcribed into text.The content analysis method was used for data analysis in Nvivo12,and data collection and analysis were carried out simultaneously.Part Ⅱ:Module content construction of WeChat applet and pre-experimentBased on the previous research results,the each module and its specific functions of the WeChat applet for home-based pulmonary rehabilitation in patients after lung cancer surgery were constructed.Twenty-two postoperative patients with lung cancer were selected as the research subjects for preliminary experiments,including 12 in the intervention group and 10 in the control group.The control group received routine nursing,and the intervention group implemented the module content intervention of the WeChat applet based on routine nursing.Exercise capacity,quality of life,dyspnea,anxiety,and depression were measured in the two groups on the second postoperative day and the day of discharge,and satisfaction in the intervention group was measured on the day of discharge.SPSS 25 software was used to process and analyze the data.If the measurement data conformed to the normal distribution,it was expressed as the mean ± standard deviation.Results:Part Ⅰ:A total of 17 respondents were included in the qualitative interview.According to the interview and analysis results,a total of 5 themes were obtained:in-hospital pulmonary rehabilitation,Barriers to home-based pulmonary rehabilitation,Have you ever used mHealth,WeChat applet functions demand,WeChat applet forms demand,and 15 sub-themes:promote expectoration,Pain management,dietary guidance,inconvenience,lack of knowledge,used,never used,breathing training,exercise training,health education,nutrition and dietary guidance,psychological counseling,counseling services,video,sound recording or text.Part Ⅱ:1.The research team named the WeChat applet "Pulmonary Rehabilitation Platform".The pulmonary rehabilitation platform includes five main sections:homepage,questionnaire filling,check-in,about us,and personal center.The initial page of entering the applet is the homepage page.It has six functional modules:exercise training,breathing training,nutrition and diet,health education,psychological counseling,and data measurement.2.The pre-experiment results showed that the intervention group could significantly improve the depressive symptoms of the patients compared with the control group,but there was no significant statistical difference between the two groups in exercise ability,quality of life,dyspnea,and anxiety.There were statistically significant differences in exercise capacity,quality of life,dyspnea,anxiety,and depression in the intervention group before and after the intervention.Patients in the intervention group were generally satisfied with the content of each module of this WeChat applet.Conclusion:1.Patients after lung cancer surgery have a certain understanding of pulmonary rehabilitation.Inconvenience and lack of knowledge are the main resistance factors for patients to perform home-based pulmonary rehabilitation.At present,patients do not generally use mobile medical-related websites,APPs,or small programs and other services.The patients put forward suggestions on the function and form of the homebased pulmonary rehabilitation WeChat applet and expressed their willingness to use this WeChat applet for home pulmonary rehabilitation guidance in the future.2.According to the results of demand analysis,combined with relevant literature at home and abroad,the module content of the home-based pulmonary rehabilitation model for lung cancer patients after lung cancer surgery based on WeChat applet is constructed,and the pre-experiment of each module content of the applet is carried out.The pre-experiment results showed that,compared with the control group,the intervention group could significantly improve the depressive symptoms of patients after lung cancer surgery,and the patients in the intervention group were satisfied with the content of each module of the applet,which provided a basis for the release and promotion of the WeChat applet for home pulmonary rehabilitation in the later stage.
