| [Purpose] To investigate the relationship between the histopathological features of stromal,FAP staining,E-cadherin staining with the prognosis and lymphatic metastasis in different stage colorectal carcinoma.[Methods] A retrospective study was launched with 148 patients’ s pathological specimens which were diagnosed as different stage of colorectal carcinoma and during endoscopic resection or surgery in the affiliated hospital.Their clinical characters and histopathological features were collected(including Tumor-stromal ratio,Tumor budding,morphology of CAFs at the frontline of tumor invasion,Desmoplastic reaction in the enrichment area of tumor glands,FAP immunohistochemical staining and E-cadherin immunohistochemical staining,Follow-up were collected by phone(including overall survival).All of the data were used to analysis the relationship with lymphatic metastasis,and the correlation between histopathological features and survival.[Result] 1.Among the 148 patients,survival analysis demonstrated that there were significant statistics differences between the different groups of TNM staging,tumor differentiation,TSR,tumor budding,FAP immunohistochemical staining and E-cadherin immunohistochemical staining(p<0.05);The TSR,TB and lymphatic metastasis were the independence factors of patients’ survival(Cox proportional hazards mode,p<0.05).2.The differentiation of tumor,positive FAP staining score were the independence factors of patients’ lymphatic metastasis(Logistics regression analysis,p<0.05).3.The distribution of histopathpological features in different groups of invasion depth demonstrated that all of negative FAP staining score were located in the HGIN+p T1 a group(n=11,100%).Mature morphology of CAFs were more located in the group of HGIN+p T1 a group(n=28,66.7%).In the group with the invasion depth of p T2-p T4,tumor budding above Grade1(n= 81,90%),immature DR(n=68,80%),immature morphology of CAFs(n=88,83%),FAP strongly staining(n=91,88.3%),E-cadherin negative staining(n= 42,85.7%).4.TSR,TB,morphology of CAFs,FAP staining were positive correlation with invasion depth;E-cadherin was negative correlation with invasion depth(Kendall test,p<0.05).5.There were serious differences between D2-40 and COL11A1 immunohistochemical staining and previously reported in published literature,so systematic scoring and statistical calculation could not be worked out.[Conclusion] 1.TSR,TB and lymphatic metastasis were the independence factors of patients’ survival.The differentiation of tumor and positive FAP staining score were independence factors of patients’ lymphatic metastasis.2.The patients who with TSR<1,Tumor budding above Grade1,Immature morphology of CAFs at the frontline of tumor invasion,FAP strongly positive staining,E-cadherin negative staining were under high risk of lymphatic metastasis,and they needed to be reasonable layered and managered follow-up carefully.3.TSR,TB,morphology of CAFs,FAP staining were positive correlation with invasion depth;E-cadherin staining was negative correlation with invasion depth.4.The localization,quantification and biological functions of D2-40 and COL11A1 need further experimental studies to demonstrate. |
