Objective:To evaluate the efficacy and safety of"Needling Twelve Acupoints in the Hands and Feet"on gastrointestinal function of patients with severe acute pancreatitis(SAP)in Fu-organ excess heat syndrom,to explore the application of acupuncture in the treatment of acute abdomen.Methods:110 patients who matched the inclusion criteria were randomly divided into 3groups.Patients in treatment Group A were treated with routine treatments and acupuncture of"Needling Twelve Acupoints in the Hands and Feet",Patients in control Group B received routine treatments and sham acupuncture.Patients in control Group C were treated with routine treatments only.The differences between the groups were evaluated by comparing symptom grading scale,abdominal distension pain VAS score,sense of hunger,time of spontaneous bowel movement,bowel sounds recovery time,and so on.Results:1.A total of 110 cases of SAP patients in Fu-organ excess heat syndrome were recruited,12 cases dropped out during the test,and a total of 98 patients completed the whole process of the study:33 in the group A,32 in the group B,and 33 in the group C.2.Baseline period:There were no significant differences among three groups,in onset time,APACHⅡscore,Balthazar CT score,Ranson score,VAS score of abdominal distend pain,age and gender among three groups(P>0.05).The baseline among the three groups was comparable.3.Comparison within the three groups of efficacy evaluation:there were significant improvements in three groups in symptom grading scale,VAS score,CRP concentration,IL-6 concentration,PCT concentration and serum Ca2+concentration before and after the treatment(P<0.05),while the comparisons of within groups in WBC showed no statistical significant(P>0.05).4.Comparison among the three groups of efficacy evaluation indexes:(1)In terms of gastrointestinal function indexes:(1)After 3 days of treatment,the scores of symptom grading scale of group A was lower than both of group B and group C(P<0.05),while the scores between group B and group C was not statistically different(P>0.05);After 5days of treatment,the scores of symptom grading scale were not statistically different among the three groups(P>0.05).(2)The recovery time of spontaneous bowel movement of group A was lower than both of group B and group C(P<0.05),while there was no significant difference between group B and group C(P>0.05).The differences were no statistical difference in the sense of hunger time and the bowel sounds recovery time among the three groups(P>0.05).(2)In terms of VAS scores of abdominal distend pain,On the 2nd day and 5th day of treatment,the scores of group A was lower than both of group B and group C(P<0.001),while the difference between Group B and Group C was not significantly different(P>0.05);On the 3rd day and 4th day of treatment,the scores of group A was lower than of group B,the scores of group B was lower than of group C(P<0.05).(3)In terms of laboratory indicators,comparisons between the three groups showed no statistical significance in WBC count,PCT concentration,CRP concentration and IL-6concentration(P>0.05);After 3days of treatment,the serum calcium concentration of group A was higher than both of group B and group C(P<0.05).(4)Comparing the days of hospitalization time,the hospitalization time of group was shorter than of group B(P<0.05),while here were no significant differences among group C and group A,group C and group B(P>0.05).Conclusion:SAP patients in Fu-organ excess heat syndrome are treated with routine treatments and"Needling Twelve Acupoints in the Hands and Feet"is better to promote the recovery of gastrointestinal function,and relieve abdominal pain than routine treatments only.And this complex therapy can promote the recovery of serum calcium concentration,which may slow down the development trend of the disease.
