ObjectiveTo investigate the analgesic effect of wrist ankle acupuncture on patients after transurethral plasmakinetic resection of prostate,and to compare and analyze the influence of wrist ankle acupuncture and intravenous patient-controlled analgesia pump on postoperative adverse symptoms such as wound pain,bladder spasm and bleeding;The dosage of analgesic pump,the incidence of side effects,the volume and duration of bladder irrigation fluid,and the indwelling catheterization time were measured;And the effect on urodynamic index,prostate symptom score and quality of life.MethodsIn this study,convenient cluster sampling method was used to select the patients who met the diagnostic criteria of benign prostatic hyperplasia and were scheduled to undergo transurethral plasmakinetic resection of the prostate from June 2020 to February 2021 in the Department of Urology of a top three hospital in Chengdu,Patients were given PCIA(analgesia pump group,n1=32),wrist ankle acupuncture(wrist ankle acupuncture group,n2=32),wrist ankle acupuncture combined with PCIA(combined group,n3=33).The degree of wound pain,the incidence of bladder spasm,the dosage of analgesic pump and the incidence of side effects,the volume and duration of bladder irrigation fluid,the results of hemoglobin laboratory examination on the third day after operation,the days of indwelling catheterization,the maximum urine flow rate on the first day after pulling out the catheter,the residual urine volume in the bladder,and the duration of bladder irrigation fluid were compared among the three groups Prostate symptom score,quality of life score and so on.ResultsThe results showed that there were no significant differences in age,BMI,course of disease,education level,occupation,monthly income,payment method,smoking history and drinking history among the three groups(P>0.05).The results showed that:1.There were significant differences in the degree of wound pain at 6h,24h,48h and 72h after operation among the three groups(P<0.05);There was no significant difference in the degree of wound pain among the three groups at 6h after operation(P>0.05);Results:the pain scores of the combined analgesia group were lower than those of the analgesia pump group and wrist ankle acupuncture group(P<0.05),but there was no significant difference between the two groups(P>0.05),The pain score of wrist ankle acupuncture group was lower than that of analgesia pump group 72hours after operation(P<0.05).2.The bladder spasm of the three groups at 24 h,48 h and 72 h after operation was compared,and the difference was statistically significant(P<0.05);The results showed that the degree of postoperative bladder spasm in the wrist ankle acupuncture group and the combined group was lower than that in the analgesia pump group(P<0.05),and there was no significant difference between the wrist ankle acupuncture group and the combined group(P>0.05).3.The hemoglobin laboratory test results of the three groups before and 3 days after operation were compared,and the differences were statistically significant(P<0.05);The hemoglobin levels of the three groups on the 3rd day after operation were higher than those of the analgesia pump group(P<0.05).There was no significant difference between the wrist ankle acupuncture group and the combined group(P>0.05).4.There were statistically significant differences in the dosage of analgesic pump and the incidence of side effects between the two groups(P<0.05).The dosage of analgesic pump and the total dosage within 72 hours in the combined group were lower than those in the analgesic pump group within 24 hours,24-48 hours,48-72hours after operation,The incidence of side effects in the combination group was lower than that in the analgesia pump group(P<0.05).5.There was significant difference in the volume and duration of postoperative bladder irrigation among the three groups(P<0.05);The results showed that the volume and duration of postoperative bladder irrigation fluid in the wrist ankle acupuncture group and the combined group were less than those in the analgesia pump group(P<0.05),and there was no significant difference between the wrist ankle acupuncture group and the combined group(P>0.05).6.There were statistically significant differences in postoperative indwelling catheterization days among the three groups(P<0.05);The results showed that the indwelling catheterization days of wrist ankle acupuncture group and combined group were shorter than those of analgesia pump group(P<0.05),and there was no significant difference between wrist ankle acupuncture group and combined group(P>0.05).7.There was significant difference in the recovery of bladder function among the three groups on the first day after removal of urinary catheter(P<0.05);The results showed that the maximum urine flow rate of the wrist ankle acupuncture group and the combined group were higher than that of the analgesia pump group(P<0.05),and there was no significant difference between the wrist ankle acupuncture group and the combined group(P>0.05);The bladder residual urine volume in the wrist ankle acupuncture group and the combined group was lower than that in the analgesia pump group(P<0.05),and there was no significant difference between the wrist ankle acupuncture group and the combined group(P>0.05).8.There were significant differences in prostate symptom score and quality of life score among the three groups on the first day after catheter removal(P<0.05);The results showed that the wrist ankle acupuncture group and the combined group were lower than the analgesia pump group(P<0.05),and there was no significant difference between the wrist ankle acupuncture group and the combined group(P>0.05);The results showed that the wrist ankle acupuncture group and the combined group were lower than the analgesia pump group(P<0.05),and the wrist ankle acupuncture group was lower than the combined group(P<0.05).9.The differences of hospitalization time among the three groups were statistically significant(P<0.05);Further pairwise comparison between groups,the results showed that the hospitalization time of wrist ankle acupuncture group and combined group were shorter than that of analgesia pump group(P<0.05),and there was no significant difference between wrist ankle acupuncture group and combined group(P>0.05).Conclusions1.Wrist ankle acupuncture,patient-controlled intravenous analgesia pump and wrist ankle acupuncture combined with patient-controlled intravenous analgesia pump have good analgesic effect on postoperative pain after transurethral plasmakinetic resection of prostate;The postoperative analgesic effect of wrist ankle acupuncture combined with patient-controlled intravenous analgesia pump is better than that of wrist ankle acupuncture or patient-controlled intravenous analgesia pump alone;The analgesic effect of wrist ankle acupuncture at 72 hours after operation was better than that of patient-controlled intravenous analgesia pump.2.Compared with using Wrist Ankle needle or patient-controlled intravenous analgesia pump alone,Wrist Ankle needle combined with patient-controlled intravenous analgesia pump can not only effectively reduce the degree of bladder spasm,reduce the dosage of analgesic pump and the incidence of side effects,but also help to reduce the volume and duration of bladder irrigation fluid To maintain the level of hemoglobin,promote the recovery of bladder function and urination function,reduce the indwelling catheterization time,improve the quality of life and other aspects have a positive role.
