| ObjectiveTo compare and analyze the effects of intravenous infusion of 5%GS and its dose on postoperative blood glucose level,incidence of hypoglycemia symptoms,anesthesia recovery time,postoperative vertigo,postoperative nausea and vomiting and early postoperative recovery quality in patients undergoing painless gastrointestinal endoscopy.MethodsIn this study,convenient cluster sampling method was used.Patients who received painless gastrointestinal endoscopy in digestive endoscopy center of a Grade-A hospital in Chengdu from September 2020 to December 2020 were divided into four groups according to random number table.Patients were separately given NS 300ml(0g glucose group,n1=80),5%GS 100ml+NS 200ml(5g glucose group,n2=80),5%GS 200ml+NS 100ml(10g glucose group,n3=80)and 5%GS 300ml(15g glucose group,n4=80).The vertigo score at 15min after operation,the blood glucose level at 30min after operation,the incidence of hypoglycemia,anesthesia recovery time,vertigo score,nausea,vomiting and retching score and incidence,the quality of postoperative recovery,and the incidence of nausea,vomiting and retching at 24h after operation were compared among the four groups.The test level of this study was set as 0.05,and the difference was statistically significant with P<0.05.ResultsThe general data and research related data of the four groups were compared and analyzed.There were no significant differences in age,body mass index,gender,smoking,drinking,educational level,hypertension,history of abdominal surgery,history of endoscopy,bowel preparation,fasting time,drinking time,dosage of propofol,operation time and preoperative PQRS score among the four groups(P>0.05).The basic data of the four groups were comparable.Comparative analysis of research effect evaluation indexes of four groups of patients:1.The blood glucose levels of the four groups were compared before and 30min after operation.The blood glucose levels of the 0g glucose group were lower than those before operation(P<0.05);There was no significant difference in the blood glucose level at 30min after operation between the 5g glucose group and the preoperative group(P>0.05);The blood glucose level at 30min after operation in the10g and 15g glucose groups was higher than that before operation(P<0.05).There was no significant difference in the blood glucose level of the four groups before operation(P>0.05).There was significant difference in the blood glucose level of the four groups at 30min after operation(P<0.05),further pairwise comparison between groups,the results showed that the blood glucose levels of 5g,10g and 15g glucose groups were higher than that in 0g glucose group(P<0.05),and the blood glucose levels of 10g and 15g glucose groups were higher than that in 5g glucose group(P<0.05),The level of blood glucose in 15g glucose group was higher than that in 10g glucose group(P<0.05).2.The incidence of hypoglycemia was 15.31%.There was no significant difference in the incidence of hypoglycemia among the four groups before operation(P>0.05).The incidence of hypoglycemia in the 0 g and 5 g glucose groups at 30min after operation was compared between the two groups,and the difference was statistically significant(P>0.05),the incidence of hypoglycemia in 5g glucose group was lower than that in 0g glucose group.At 30min after operation,there was no hypoglycemia in 10 g and 15 g glucose groups,but 3 cases of hyperglycemia in 15 g glucose group.3.There was significant difference in anesthesia recovery time between the four groups(P<0.05).Further pairwise comparison between groups,the results showed that the recovery time of 5g,10g and 15g glucose group was shorter than that of 0g glucose group(P<0.05);There was no significant difference in anesthesia recovery time among 5g,10g and 15g glucose groups(P>0.05).4.The vertigo scores at 15min and 30min after operation were compared among the four groups,and the differences were statistically significant(P<0.05).Further pairwise comparison between groups,the results showed that the vertigo scores of 5g,10g and 15g glucose groups were lower than those of 0g glucose group at 15min and30min after operation(P<0.05);There was no significant difference in vertigo score between 5g,10g and 15g glucose groups 15min after operation(P>0.05);At 30min after operation,the vertigo scores of the 10g and 15g glucose groups were lower than those of the 5g glucose group(P<0.05),and there was no significant difference between the 10g and 15g glucose groups(P>0.05).5.Comparison the incidence of nausea,vomiting and retching:5.1 The degree of nausea,vomiting and retching in the first 30min after operation were compared among the four groups,and the difference was statistically significant(P<0.05).Further pairwise comparison between groups,the results showed that the degree of nausea,vomiting and retching in 5g,10g and 15g glucose groups were lower than that in 0g glucose group(P<0.05);There was no significant difference in the degree of nausea,vomiting and retching among 5g,10g and 15g glucose groups(P>0.05).5.2 The incidences of nausea,vomiting and retching in the first 30min after operation were compared among the four groups,and the differences were statistically significant(P<0.05).Further pairwise comparison between groups,the results showed that the incidence of nausea,vomiting and retching in 5g,10g and 15g glucose groups were lower than that in 0g glucose group(P<0.05),and there was no significant difference in the incidence of nausea,vomiting and retching in 5g,10g and 15g glucose groups(P>0.05).5.3 There was significant differences in the incidence of nausea and retching among the four groups at 24 hours after operation(P<0.05).Further pairwise comparison between groups,the results showed that the incidence of nausea and retching in 5g,10g and 15g glucose groups were lower than that in 0g glucose group(P<0.05),and there was no significant difference in the incidence of nausea and retching in 5g,10g and 15g sugar groups(P<0.05).There was no significant difference between the four groups in the incidence of vomiting at 24 hours after operation(P>0.05).6.Comparison of early postoperative recovery quality of patients:6.1 There was significant difference in the total score of postoperative recovery quality at 30min after operation among the four groups(P<0.05).Further pairwise comparison between groups,the results showed that the total scores of postoperative recovery quality in 5g,10g and 15g glucose groups were higher than those in 0g glucose group(P<0.05);The total score of postoperative recovery quality in 10g and15g glucose groups was higher than that in 5g glucose group(P<0.05);However,there was no significant difference in the total score of postoperative recovery quality between the 10g and 15g glucose groups(P>0.05).6.1.1 There was no significant difference in the scores of physiological factors at30min after operation among the four groups(P>0.05).6.1.2 There was significant difference in the scores of perceived nociceptive factors among the four groups at 30min after operation(P<0.05).Further pairwise comparison between groups,the results showed that the scores of nociceptive factors in 5g,10g and 15g glucose groups were higher than those in 0g glucose group(P<0.05);There was no significant difference in nociceptive factor scores among 5g,10g and 15g glucose groups(P>0.05).6.1.3 There was no significant difference in the scores of emotional factors among the four groups at 30min after operation(P>0.05).6.1.4 There was significant difference in the scores of activities of daily living at30min after operation among the four groups(P<0.05).Further pairwise comparison between groups,the results showed that the scores of activities of daily living in 5g,10g and 15g glucose groups were higher than those in 0g glucose group(P<0.05);There was no significant difference in activities of daily living score among 5g,10g and 15g glucose groups(P>0.05).6.1.5 The cognitive factor scores of the four groups at 30min after operation were compared,and the difference was statistically significant(P<0.05).Further pairwise comparison between groups,the results showed that the scores of cognitive factors in 10g and 15g glucose groups were higher than those in 0g and 5g glucose groups(P<0.05);There was no significant difference in cognitive factor scores between 10g and 15g glucose groups(P>0.05);There was no significant difference in cognitive factor scores between 0g and 5g glucose groups(P>0.05).6.2 There was significant difference in the total recovery rate of the four groups at 30min after operation(P<0.05).Further pairwise comparison between groups,the results showed that the total recovery rates of 5g,10g and 15g glucose groups were higher than 0g glucose group(P<0.05).The total recovery rates of 10g and 15g glucose groups were higher than 5g glucose group(P<0.05).There was no significant difference in the total recovery rates of 10g and 15g glucose groups(P>0.05)6.2.1 There was no significant difference in the physiological recovery rate of the four groups at 30min after operation(P>0.05).6.2.2 There was significant difference in the recovery rate of nociception among the four groups at 30min after operation(P<0.05).Further pairwise comparison between groups,the results showed that:5g,10g and 15g glucose group patients’nociceptive recovery rate were higher than 0g sugar group(P<0.05),5g,10g and 15g glucose groups patients’nociceptive recovery rate was no significant difference(P>0.05).6.2.3 There was no significant difference in the emotional recovery rate of the four groups at 30min after operation(P>0.05).6.2.4 There was significant difference in the recovery rate of activities of daily living among the four groups at 30min after operation(P<0.05).Further pairwise comparison between groups,the results showed that:5g,10g and 15g glucose group of patients with activities of daily living recovery rate were higher than 0g glucose group(P<0.05),5g,10g and 15g glucose group of patients with activities of daily living recovery rate was no significant difference(P>0.05).6.2.5 There was significant difference in the recovery rate of cognitive function among the four groups at 30min after operation(P<0.05).Further pairwise comparison between groups,the results showed that the cognitive recovery rates of 5g,10g and 15g glucose groups were higher than that of 0g glucose group(P<0.05),the cognitive recovery rates of 10g and 15g glucose groups were higher than that of 5g glucose group(P<0.05),and there was no significant difference between 10g and 15g glucose groups(P>0.05).Conclusions1.Intravenous infusion of 5%GS is helpful to prevent postoperative hypoglycemia,reduce the incidence of postoperative hypoglycemia,shorten the recovery time of anesthesia,relieve postoperative vertigo,reduce the degree of postoperative nausea,vomiting and retching,and improve the quality of early postoperative recovery.2.The incidence of preoperative hypoglycemia in patients undergoing painless gastrointestinal endoscopy was 15.31%.3.Intravenous infusion of 5%GS 100m L can maintain the postoperative blood glucose level;At 30min after operation,intravenous infusion of 5%GS 200m L or300m L was better than 5%GS 100m L in reducing the incidence of hypoglycemia,alleviating vertigo symptoms and improving the quality of early postoperative recovery;Compared with intravenous infusion of 5%GS 300m L,intravenous infusion of 5%GS 200m L in patients with postoperative 30min blood glucose level tends to be more safe range,suitable for clinical application. |
