Objective: To explore the usefulness of contrast-enhanced ultrasound in the diagnosis of benign and malignant orbital tumors.Methods: This study is a retrospective non-random cohort study.30 cases of various orbital tumors,whose pathological results were finally obtained in the Department of Ophthalmology at the First Affiliated Hospital of Hunan Normal University between January 2019 and January 2021,were selected and analyzed by routine ultrasound and ultrasound contrast data.Color Doppler ultrasound diagnostic equipment with real-time ultrasound contrast matching imaging technology were used before surgery.Contrast enhancement observations were conducted.The ultrasound contrast agent was SonoVue from Bracco,Italy,and its main component was sulfur hexafluoride(SF6).Enhanced observation lasted for 180 seconds after rapid injection through the superficial vein of the forearm.The same experienced ultrasound doctor diagnosed the ultrasound image,and recorded the perfusion of the contrast agent in the tumor,the uniformity of internal echo,the degree of enhancement and the stage of enhancement.The same doctor combined the results of pathological tissue to further determine the benign and malignant tumors.Results:According to the pathological study of orbital tumors,among 30 cases of orbital tumors,21 cases were diagnosed as benign and 9 cases were malignant.Among the 30 cases of orbital tumors,19 cases of contrast-enhanced ultrasound were benign and 11 cases were malignant.There are two cases that contrast-enhanced ultrasound diagnoses did not match pathological diagnoses.These two cases are inflammatory granuloma.Comparing with pathological results,the diagnostic sensitivity,specificity,accuracy of contrast-enhanced ultrasound were 100.00%(9/9),90.47%(19/21)and 93.33%(28/30),respectively.Cystic lesions only showed peripheral enhancement after contrast enhancement,but no internal enhancement.The main characteristics of ultrasound examination of benign tumors after contrast enhancement were fast forward and slow retreat,uniform or low enhancement.Malignant tumors generally were fast forward and fast retreat,uneven and high enhancement.Conclusions:1.Contrast-enhanced ultrasound and pathological examination have a high degree of consistency(Kappa=0.85).Contrast-enhanced ultrasound helps to distinguish benign and malignant tumors before surgery.2.Contrast-enhanced ultrasound provides a more effective diagnostic basis.It can be applied to distinguish benign and malignant tumors and other eye diseases before clinical surgery,select surgical methods and conduct prognostic evaluation.3.The contrast-enhanced ultrasound technology effectively improves the detection rate and coincidence rate of orbital tumors,and reduces the misdiagnosis rate.
