Effect Of Cognitive Therapy Combined With Music Relaxation Therapy On Couples With Female Infertility Undergoing In Vitro Fertilization And Embryo Transfer

Objective:Under the guidance of psychological stress theory,this study explored the effect of cognitive therapy combined with music relaxation therapy on the psychological status and sleep quality of infertile couples undergoing IVF-ET.Methods:From June to October 2020,132 female infertility patients were selected from a reproductive hospital in Changsha,Hunan Province.The subjects were randomly assigned to the control group,female intervention group and couple intervention group,Interventions were performed at six time points(the day of filing,the day of ovulation induction,the sixth day of ovulation induction,the day of HCG injection,thirty minutes after oocyte retrieval,an hour after transplantation)in the IVF-ET cycle.The female intervention group was given cognitive music relaxation therapy,the couple intervention group took cognitive music relaxation therapy for women,and their husbands were cognitive intervention,and the control group was given routine nursing.The self-regulation fatigue level,coping style,social support and sleep status were assessed before and after intervention,and the intervention effect was compared.Results:1.There was no statistically significant in general demographic data,self-regulation fatigue score,trait coping style score,sleep quality score and social support score among the three groups before intervention(P >0.05).2.Comparison of Self regulated fatigue level of the three groups before and after the intervention:(1)After the intervention,the total scores of self-regulation fatigue in the control group,female intervention group and couple intervention group were(49.71± 4.07),(47.39± 3.86)and(44.91 ± 3.61)respectively.In each dimension and total score of self-regulation fatigue among the three groups,the difference was statistically significant(P < 0.05).The difference of self-regulation fatigue between the three groups before and after intervention was statistically significant(P < 0.05).Post test further showed that the decrease of self-regulation fatigue score in the control group < female intervention group < couple intervention group.(2)Before and after the intervention,there was no significant difference in the self-regulation fatigue score of the control group(P > 0.05).The score of self-regulation fatigue in female intervention group after intervention was(47.39±3.86),which was lower than that before intervention(48.75±3.52),and the self-regulation fatigue score of the couple intervention group(44.91 ±3.61)was lower than that before the intervention(47.91±3.52),the differences were statistically significant(P < 0.05).3.Comparison of coping style scores of three groups before and after the intervention:(1)After intervention,the positive response scores of control group,female intervention group and couple intervention group were(35.49 ± 3.68),(35.20 ± 3.30),(35.19 ± 2.76),and there was no statistical significance(P > 0.05),and negative response scores were(28.11 ± 4.15),(24.50 ± 3.89),(22.26 ± 3.49),and the difference was statistically significant(P < 0.05).The difference of negative coping between the three groups before and after intervention was statistically significant(P < 0.05).The comparison of negative coping scores further showed that: the negative coping score of the control group decreased<female intervention group < couple intervention group.(2)Before and after the intervention,there was no significant difference in the positive coping score and negative coping score of the control group(P > 0.05),and there was no significant difference in the positive coping score of the female intervention group and the couple intervention group after the intervention(P > 0.05).The score of negative coping in female intervention group after intervention was(24.50±3.89),which was lower than that before intervention(27.41 ±4.46),and the scores of negative coping in the couple intervention group(22.26±3.49)were significantly lower than those before the intervention(28.53±4.60),the differences were statistically significant(P < 0.05).4.Comparison of the scores of social support of the three groups before and after the intervention:(1)After the intervention,the total scores of social support in the control group,the female intervention group and the couple intervention group were(25.89 ± 3.66),(28.11 ± 5.31)and(30.33±3.82),respectively.In each dimension and total score among the three groups,the difference was statistically significant(P < 0.05).The difference of social support between the three groups before and after intervention was statistically significant(P < 0.05).Pairwise comparison further showed that: the rising degree of social support score in the control group < female intervention group < couple intervention group.(2)Before and after the intervention,there was no significant difference in the scores of social support in the control group(P > 0.05).The score of social support in female intervention group after intervention was(28.11±5.31),which was higher than that before intervention(25.18±4.74),and the scores of social support in the couple intervention group(30.33±3.82)were higher than those before the intervention(25.86±3.32),the differences were statistically significant(P < 0.05).5.Comparison of sleep quality scores of the three groups before and after the intervention:(1)After the intervention,the sleep quality scores of the control group,the female intervention group and the couple intervention group were(22.64±4.62),(20.59±4.86)and(17.84±3.85)respectively,and the difference was statistically significant(P <0.05).The difference of sleep quality scores between the three groups before and after intervention was statistically significant(P < 0.05).Pairwise comparison further showed that: the decrease of sleep quality score in the control group < female intervention group < couple intervention group.(2)Before and after the intervention,there was no significant difference in the sleep quality score of the control group(P >0.05).The score of sleep quality in female intervention group after intervention was(20.59 ± 4.86),which was lower than that before intervention(24.45 ± 4.70),and the sleep quality score of the couple intervention group(17.84 ± 3.85)was lower than that before the intervention(23.81±3.76),the differences were statistically significant(P < 0.05).Conclusion:Cognitive therapy combined with music relaxation therapy can effectively improve IVF-ET women’s self-regulation fatigue and negative coping style,improve the level of social support and sleep quality.And the intervention effect of couples is better than that of women alone.
