Objective:To looking for the harmful factors of acute myocardial infarction(AMI)in young people in this area,so as to strengthen the screening and primary prevention of the high-risk group,reduce the sickness rate of AMI in young people,and provide evidence for therapy decision-making by analyzing the trait of clinical and coronary arterylesions.Method:Through the retrospective analysis method,choose AMI patients treated in our hospital from January 2016 to December 2020,grouped according to age,youth(age≤44 years old)in patients with a total of 82 cases of observation group,randomly over the same period of the diagnosis of AMI in elderly patients(age ≥ 60 years old)98 cases as control group.Medical records of patients in the two groups were collected,including previous medical history,clinical symptoms,causes of disease and other clinical data,biochemical examination,electrocardiogram,coronary angiography.Compare the differences between the two groups.Results:1.Compared with the elderly group,the constituent ratio of smoking,overweight or obesity,male patients and family history of early onset coronary heart disease were higher in the youth group(P<0.05),while hypertension and diabetes were relatively low(P<0.05).2.The TC,TG,LDL-C level was higher in the youth group than matched group(P<0.05),hyperlipidemia and HDL-C level were roughly the same in both groups(P>0.05).3.Smoking,overweight or obesity,TG,LDL-C levels,family history of early coronary were independently positively correlated with youth AMI(P<0.05).4.Compared with the elderly group,the proportion of typical chest pain symptoms,alcoholism,overwork and staying up late in youth group was higher,while the proportion of previous angina pectoris was relatively low,and the rate of STEMI was higher in the young group(P<0.05).5.Compared with the control group,the ratio of single-vessel lesion was the highest and that of multi-vessel lesion was the lowest in the young group(P<0.01).The proportion of LAD in the AMI group of young people was the highest.High thrombosis load,tubular stenosis and localized stenosis were higher in the youth AMI group,while collateral circulation and diffuse stenosis were relatively low(P<0.05).Conclusions:1.High AMI incidence in young men.Smoking,family history,overweight or obesity,LDL-C and TG levels are independent risk factors for youth AMI.2.Young AMI patients mainly show ST segment elevation myocardial infarction,some of them have alcoholism,overwork and stay up late before the onset of the disease,most of them have typical chest pain symptoms.The previous angina pectoris attack is rare before the onset of the disease.3.Young AMI coronary artery disease is mainly single vessel,the proportion of anterior descending branch is the highest.High thrombosis load,tubular stenosis and localized stenosis are common.But few collateral circulation is formed,and diffuse stenosis is relatively rare.
