Correlation Between Soluble ST2 And Pulmonary Hypertension Due To Left Heart Hailure

ObjectiveIn this study,we selected some left heart failure patients which with pulmonary hypertension or not for the research object.The serum soluble ST2 level in these patients were detected.And,we analyzed the dynamic changes and relationship of the serum soluble ST2 level in these patients.Then investigated the correlation between serum level of soluble ST2 and pulmonary artery pressure.MethodsBy using of case-control method,a total of 116 patients hospitalized in Liaocheng People’s Hospital from October 2017 to August 2018 who were diagnosed with left heart failure were enrolled.None of these patients were complicated with diabetes,malignant tumor,acute coronary syndrome,infectious disease or connective tissue disease.And these patients without obvious liver dysfunction or kidney dysfunction.In addition,we also needed exclude idiopathic artery pulmonary hypertension and patients which with pulmonary hypertension due to chronic obstructive pulmonary disease,embolic disease,portal vein disease,autoimmune diseases,infectious diseases,drugs or poison induced pulmonary hypertension.Informed consent was signed for all patients.After symptomatic remission on the second day after admission,pulmonary artery pressure was measured by echocardiography.According to their pulmonary artery pressure,26 patients with normal pulmonary artery pressure were in the control group(group A: PASP≤35mm Hg),and 90 patients with elevated pulmonary artery pressure were experimental group.Then the experimental group was further divided into mild pulmonary hypertension group(group B:35mm Hg<PASP≤50mm Hg)and moderate to severe pulmonary hypertension group(group C: PASP>50mm Hg).At the same time,the patient’s fasting elbow venous blood was taken to the coagulation tube,centrifuged in a centrifuge,the upper serum was aspirated into the EP tube and stored in a refrigerator at-80℃.The serum soluble ST2 level was determined by enzyme linked immunosorbent assay.At the same time,The concentration of NTpro BNP,hs-CRP,RDW-SD,RDW-CV,FBG and Hb was detected.Results1.The serum soluble ST2 levels in group B and C were higher than those in group A(p<0.05,p<0.01).The serum soluble ST2 level in group C was higher than that in group B(p<0.05).With the increase of pulmonary artery pressure,the serum soluble ST2 level of the patient with heart failure increased accordingly;2.There were no differences in age,sexuality,smoking history,drinking history,FBG or Hb among the groups A,B,and C(p>0.05).The levels of NT-pro BNP,hs-CRP,RDW-SD and RDW-CV in group C were higher than those in group A and B(p<0.05).While there were no significant differences in levels of NT-pro BNP,hs-CRP,RDW-SD and RDW-CV between group A and B(p > 0.05).3.Pearson linear correlation analysis and multiple linear regression analysis showed that the levels of serum soluble ST2 were positively correlated with pulmonary artery pressure and levels of NT-pro BNP,hs-CRP,RDW-SD,RDW-CV(p<0.05).But,there were not correlation between the level of serum soluble ST2 and FBG,Hb(p>0.05).ConclusionsThe level of serum soluble ST2 in patients which with pulmonary hypertension due to left heart failure was significantly elevated,and the level of serum soluble ST2 increased with the increase of pulmonary artery pressure.These was a correlation between the level of serum soluble ST2 and pulmonary artery pressure.SignificanceThe serum soluble ST2 was a novel marker of cardiac failure.These was a certain predictive value for the prognosis of patients with heart failure.However,it had not been paid attention to in clinical practice and the prevalence rate was low.This study further observed the relation between the level of serum soluble ST2 and the severity of heart failure and left heart failure in patients with pulmonary hypertension.And showed that the soluble ST2 is associated with the severity of left heart failure and pulmonary hypertension due to left heart failure.The study would increases the understanding of soluble ST2 by the medical staff and conducive to the promotion of soluble ST2 in clinical practice.It was shown the serum soluble ST2 contributed to risk stratification and prognosis assessment of heart failure.The serum soluble ST2 could become one of the important biomarkers for future diagnosis and treatment of heart failure and assessment of cardiac fibrosis.
