Since the 21 st century,with the development of economy,the improvement of people’s living standards,the deepening of the problem of aging population,the people’s demand for medical services has been further strengthened,medical services as a special type of service,both the characteristics of general services,but also different from the characteristics of general services.Optimizing the performance of hospital medical service is the focus of continuous promotion and development in the process of hospital reform,hospital as the main body of medical service performance appraisal and implementation,the department as the basic unit to provide medical services to patients,using scientific and reasonable methods to measure and evaluate it,can further improve the core competitiveness of the hospital itself and the utilization rate of medical resources,so as to achieve the comprehensive improvement of medical level,operation management level and other aspects.And because the disease itself has the characteristics of complexity and difference,the traditional medical service performance evaluation index has certain limitations,the assessment index is directly related to whether the assessment results can be effectively applied,therefore,the performance of medical service needs to be more scientific measurement and evaluation.Since the State Council issued guidance on the comprehensive reform of urban public hospitals in 2015,the State has strongly advocated the introduction of DRGs(diagnosis-related groups)into the system of performance evaluation indicators of public hospitals,and the essence of DRGs is a clustered case combination method,that is,the use of statistical control theory to classify inpatients.DRGs have two main roles,one is to evaluate clinical performance and the other is to pay for health care.At present,clinical performance evaluation has become a more mature application of DRGs,dr Gs implementation and application can further strengthen the accuracy and efficiency of hospital accounting,provide objective data support for hospital performance evaluation,so that hospitals can objectively evaluate medical service performance from the multi-dimensional dimensions of medical service capacity,medical service efficiency and medical service safety,so as to improve the competition mechanism and management level of hospitals,make the quality of medical care continuously improve,and promote the improvement of the new management model of medical reform.On the basis of the existing literature,this thesis selects a tertiary B hospital in Zhejiang Province as the research object from DRGs measuring medical service performance as the entry point.First of all,according to the relevant literature base at home and abroad to carry out service performance evaluation related concepts of the definition and theoretical basis of the introduction.Secondly,according to the Zhejiang Statistical Yearbook,Zhejiang Health and Family Planning Statistics,DRGs related websites and the relevant data of Z Municipal Bureau of Statistics and the Health Planning Bureau,the hospital’s medical service performance evaluation is described as a descriptive statistical analysis,and the hospital’s2016-2020 DRGs medical service performance evaluation data are used for the overall regression analysis,describing future development trends and characteristics,and finding that the increasing medical expenses are still a serious problem in China’s medical insurance environment.Then,to build an inter-departmental medical service performance evaluation system based on DRGs,using the hospital’s 2020 full-year discharge patient case home page data and in-hospital DRGs related platform data,using the weighted TOPSIS method and the comprehensive index method,from the service capacity,service Efficiency and medical safety3 dimensions of the hospital between the medical service performance measurement analysis,and the use of traditional indicators and DRGs indicators of the hospital’s key disease areas of the performance of medical services measured and compared analysis,found that the use of traditional indicators and DRGs The comprehensive evaluation results of the index are not consistent,finally,through the structural analysis of case grouping and inter-departmental disease difficulty,to understand the distribution of the disease,and from the aspects of hospitalization time,related costs,difficulty of diagnosis and treatment,using the gray correlation analysis method to study the impact factors of the performance evaluation of medical services between the departments,the study found that the hospital has unreasonable disease structure,and the average number of hospital days,costs are the main factors affecting the relative weight of the disease group(RW)and case combination index(CMI).Based on the results of the study,this thesis discusses how to measure the performance of medical services between hospital departments scientifically and reasonably based on DRGs combined with health insurance-related data,and analyzes the possible problems of DRGs in the application of medical service performance evaluation,and makes targeted suggestions.
