Objective: Malignant melanoma(MM)is a malignant tumor of melanocytes originating from the skin and other organs,with the incidence increasing year by year in different parts of the world.The characteristics of MM and the incidence vary among different races.The current research on MM is mainly based on Caucasian populations,and there is a lack of studies from large data in China.The purpose of this paper is to analyze the clinical characteristics and prognostic factors of 316 cases of MM in two hospitals in Chongqing,to provide a basis for exploring the morbidity characteristics of MM in China,and to further improve the understanding and diagnosis and treatment of this disease.Methods: Clinical data of 316 patients with MM from January 2013 to December 2017 were collected from two hospitals in Chongqing,and all patients were diagnosed by pathological histology.The clinical data collected in the database included age,gender,TNM stage,primary site,presence of ulceration in the primary site,whether surgery was performed,and adjuvant treatment,etc.Overall survival(OS)was calculated for different subgroups of patients and risk factors for patient death were determined.Statistical analysis was performed using SPSS version 20.0statistical software,and survival curves were plotted using Kaplan-Meier.Univariate and multifactorial analyses of prognosis were performed using the COX regression risk model,and P<0.05 was considered statistically significant.Results: 1.Of the 316 MM patients classified by gender,181(57.27%)were male and 135(42.73%)were female.Of the 316 MM patients classified by age,198(62.66%)were ≤65 years old and 118(37.34%)were >65 years old.Of the 316 MM patients classified by site of onset,150(47.47%)cases were originated from skin,55(17.41%)from the eye,56(17.72%)from the nose,44(13.92%)from other mucous membranes,and11(3.48%)from other sites.Among the skin sites,105(70.00%)MM occurred in the hands and feet,17(11.33%)in the extremities,16(10.67%)in the trunk,and 12(8.00%)in the head and neck.209(66.14%)of the 316 patients with MM had stage I/II/III TNM and 107(33.86%)had stage IV.Among the 316 patients with MM,40(26.67%)had no ulceration at the primary site of the skin and 110(73.33%)had ulceration.243(76.90%)of the 316 patients with MM had surgery and 73(23.10%)had no surgery.2.The adjuvant treatment modalities among 316 MM patients were mainly divided into interferon,conventional treatment(radiotherapy,chemotherapy)and biologic targeted therapy,with the numbers of cases being 25(7.91%),76(24.05%)and 41(12.97%),respectively.Adjuvant therapy was not performed in 174(55.06%)patients.3,The median survival of 316 patients with MM was 30.40 months,with 1-,2-,and 5-year survival rates of 84.48%,59.13%,and 22.02%,respectively.Univariate prognostic analysis showed that age,clinical stage,primary site,presence or absence of ulceration at the primary skin lesion,presence or absence of surgery,presence or absence of conventional treatment and presence or absence of adjuvant treatment were correlated with the prognosis of patients(P<0.05).COX regression multifactor analysis showed that clinical stage,presence or absence of ulceration at the primary skin lesion and presence or absence of surgical treatment were independent prognostic risk factors for MM.Conclusion: 1.There were slightly more men than women with MM in the two hospitals in Chongqing,with an overall high prevalence age of60-69 years and a majority of stage I/II/III patients at initial diagnosis.the most common site of MM was the extremity,and lesions without ulcers were more common than those with ulcers.2.MM patients receiving surgical treatment were more common in the two hospitals in Chongqing,but fewer patients received adjuvant treatment than those who did not.More patients received conventional treatment(radiotherapy)in adjuvant treatment.3.Clinical stage,presence or absence of ulceration at the primary skin site,and presence or absence of surgical extended excision were independent prognostic risk factors for overall survival of MM patients.
